摘 要:目的:探讨肝细胞癌(HCC)组织中NF-kBp65,VEGF和MMP-9的表达及其临床意义。方法:应用免疫组织化学(SP法)检测35例HCC组织,15例癌旁肝组织及10例正常肝组织中NF-kBp65,VEGF和MMP-9蛋白的表达并分析其与临床病理因素的关系。结果:NF-kBp65,VEGF和MMP-9蛋白在HCC组织中高表达率(分别为80%,74%,71%)高于对照组(P<0.01)。NF-kBp65的表达与HCC的Edmondson分级有关,即在Edmondson分级Ⅲ-Ⅳ的癌细胞中NF-kBp65的表达强于Edmondson分级Ⅰ-Ⅱ的(P<0.05);有静脉浸润的HCC组织中NF-kBp65,VEGF和MMP-9的表达高于无静脉浸润者,差异有统计学意义(P<0.05)。但NF-kBp65的表达与年龄,性别,肿瘤大小,AFP水平无关(P>0.05);VEGF和MMP-9的表达与年龄,性别,肿瘤大小,AFP水平,HCC的Edmondson分级无关(P>0.05)。NF-kBp65的表达与VEGF和MMP-9呈正相关(rs1=0.579,P1<0.05;rs2=0.406,P2<0.05)。结论:NF-kBp65,VEGF和MMP-9过表达可能共同参与HCC的演进,联合检测NF-kBp65,VEGF和MMP-9可作为判断HCC恶性程度和预后的指标。Objective:This study aims to investigate the expression and significance of NF-kBp65,VEGF and MMP-9 in human hepatocellular carcinoma (HCC) progression and prognosis. Methods:The expression of NF-kBp65,VEGF and MMP-9 in 35 cases of HCC, 15 cases of adjacent non-tumor liver tissue, 10 cases of normal liver tissue were studied by immunohistochemical method (SP method), and their relationship to clinicopathological characteristics were analyzed.Results:The high expression rate of NF-kBp65, VEGF and MMP-9 in HCC tissue were 80%, 74% and 71% respectively, both higher than those in adjacent non-tumor liver tissue and normal liver tissue (P0.01). The expression of NF-kBp65 was correlated with HCC Edmondson's grade. NF-kBp65 expression in HCC Edmondson's grade Ⅲ-Ⅳ was significantly higher than that in HCC Edmondson's grade Ⅰ-Ⅱ (P〈0.05). The expression of NF-kBp65, VEGF and MMP-9 in HCC with venous invasion were significantly higher than those in HCC without venous invasion (P〈0.05). But the expression of NF-kBp65 was not correlated with age, gender, tumor size and AFP level (P〈0.05), the expression of VEGF and MMP-9 were not correlated with age, gender, tumor size, AFP level and HCC Edmondson's grade (P 〉0.05). The expression of NF-kBp65 was positively related with VEGF and MMP-9 expression in HCC(rs1=0.579, P1〈0.05; rs2=0.406, P2〈0.05). Conclusions: The high expression of NF-kBp65,VEGF and MMP-9 may be together involved in the development and metastasis of HCC. It may be a good marker for monitoring the malignant degree and the prognosis of HCC by combining NF-kBp65,VEGF and MMP-9.
