溧阳市胃癌前病变危险因素病例-对照研究

摘　　要：目的探讨胃癌前病变可能的影响因素。方法对江苏省溧阳市胃癌前病变患者进行病例-对照研究,资料处理采用单因素及多因素Logistic回归分析。结果多因素非条件Logistic回归分析结果显示,对江苏省溧阳市胃癌前病变有影响的危险因素有上消化道肿瘤家族史(OR=2.57,95%CI=1.20～5.51),常食用油条(OR=3.53,95%CI=1.66～7.51),常食用咸菜(OR=5.38,95%CI=2.19～13.21),常食用稀饭(OR=2.62,95%CI=1.06～6.46)。在调整年龄、家族遗传史、饮酒状况等混杂因素后,男性人群吸烟的OR为2.25(95%CI=1.09～4.61)。结论胃癌前病变的影响因素与胃癌类似。有上消化道肿瘤家族史的对象可能是胃癌前病变的高危人群。少摄入咸菜、油条和热烫的稀饭,可能会对胃癌前病变甚至胃癌的发生有预防作用。吸烟可能与男性胃癌前病变有关。Abstract： Objective To investigate the possible factors for gastric precancerous lesions in Liyang city, Jiangsu province. Methods Patients with precancerous lesions of gastric cancer and comparable control population were taken into this case-control study. Logistic regression analysis was conducted in data analysis process. Results Multivariate non-conditional Logistic regression analysis showed that the risk factors of gastric precancerous lesions, included ： family history of upper gastrointestinal tumors （ OR = 2.57, 95% CI = 1.20 - 5.51 ）, often intake deep-fried dough sticks （ OR = 3.53, 95% CI = 1.66 - 7.51 ）, regular consumption of pickles （ OR = 5.38, 95% CI = 2.19 - 13.21 ）, regular consumption of rice （OR =2.62, 95% CI = 1.06 -6.46）. For the male population, OR is 2.25 （95% CI = 1.09 -4.61）, after adjustment for some confounding factors as age, alcohol drinking status, family cancer history and soon. Conclusion Risk factors are familiar for both gastric cancer and gastric precancerous lesions. Population with a family history of upper digestive tract may be the risk of gastric precancerous lesions. To protect population from gastric precancerous lesions and gastric cancer, less intake of deep-fried dough sticks, pickles and hot porridge should be promoted. Smoking may be the risk factor for the male population.
