十二指肠球部溃疡患者幽门螺杆菌CagA与血清IL-17及IL-23的关系

摘　　要：目的探讨十二指肠球部溃疡(DU)患者幽门螺杆菌(Heplicobacter pylori,Hp)细胞毒素相关基因CagA与血清IL-17、IL-23的关系。方法用ELISA法检测66例Hp感染DU患者(DU组)的CagA-IgG和血清IL-17、IL-23水平,并与无临床症状的Hp感染者(AS组,40例)及健康者(对照组,40例)进行比较。结果DU组患者血清IL-17水平显著高于AS组和对照组,差异有统计学意义(P<0.05)。DU组CagA+患者血清IL-17水平〔(10.8±5.8)pg/ml〕显著高于CagA-IgG-患者〔(7.6±4.7)pg/ml〕,差异有统计学意义(P<0.05)。DU组和AS组血清IL-23水平显著高于对照组(P均<0.05),但是两组CagA-IgG+者血清IL-23水平与CagA-IgG-者比较,差异均无统计学意义(P>0.05)。结论Hp感染者血清IL-17、IL-23水平明显高于健康者,DU患者血清IL-17表达受CagA影响。Objective To investigate the relationship of virulence factor cytotoxin - associated gene （CagA） to the levels of serum IL - 17 and IL - 23 in duodenal ulcer （DU） patients. Methods CagA - IgG and serum IL - 17, IL - 23 were detected by ELISA in 66 DU patients with lip infection, and compared with those of Hp - infected asymptomatic （AS） carriers （n =40） and of healthy subjects （n =40）. Results Serum IL- 17 levels were significantly higher in DU group than in groups AS and control （P 〈0. 05）. In DU group, serum IL - 17 level was higher in patients with CagA^＋ （ 10. 8 ± 5. 8 pg/ml） than those with CagA^- （7.6 ±4. 7 ） pg/ml）, the difference was significant （P 〈0. 05 ）. But no significant difference was noted in IL - 23 level between CagA^＋ and CagA^- patients （ P 〉 0. 05 ）. Conclusion Serum IL - 17 and IL - 23 levels are remarkably higher in Hp - infected patients than in healthy subjects. Serum IL - 17 expression is influenced by CagA in DU patients.
