p21在食管鳞状细胞癌及食管鳞状上皮良性增生性病变中表达的临床意义

摘　　要：目的:探讨p21在食管鳞状细胞癌及食管鳞状上皮良性增生性病变中的表达及其意义。方法:对75例食管鳞状细胞癌标本、15例食管鳞状上皮良性增生性病变及10例正常鳞状上皮标本的石蜡切片进行免疫组织化学标记,用S-P法检测p21的表达。结果:p21在食管正常组织中无表达,在食管鳞状上皮良性增生性病变中阳性率为33.33%,在食管鳞状细胞癌组织中阳性率为78.87%,差异有统计学意义(P<0.01);p21在食管鳞状细胞癌的高分化组、中分化组与低分化组阳性率分别为68.89%、90.00%和100.00%,差异有统计学意义(P<0.05)。结论:检测p21的表达,可以了解食管癌组织的分化程度,指导临床治疗和判断预后,而且p21可以作为一种可靠的标志物,对食管癌的早期诊断具有重要价值。Objective：To investigate the expression and significance of p21 protein in the tissue of esophageal cancer and benign esophageal hyperplasia.Methods：Seventy-five cases of esophageal cancer specimen,15 benign esophageal hyperptasia specimen and 10 normal specimen were performed HE staining before immunohistochemistry analysis,and the expression of p21 protein was examined by immunohistochemical（S-P method）.Results：The expression of p21 protein was negative in normal tissue,33.33% in benign esophageal hyperplasin tissue and 78.87% in esophageal cancer.The difference had statistical significance（P0.01）.The positive expression of p21 protein was 68.89% in high differential group,90.00% in medium differential group and 100.00% in low differential group.The difference was statistically significant（P0.05）.Conclusions：The expression of p21 protein may demonstrate the differential degree of esophageal cancer,guide the clinical therapy and help to judge the prognosis.P21 protein is regarded as a reliable label in early diagnosis of esophageal cancer.
