血清Livin水平与胃癌临床分期的关系探讨

摘　　要：目的探讨血清Livin水平与胃癌临床分期的关系。方法用ELISA法检测36例术前胃癌患者和5例健康人血清Livin水平。结果胃癌患者血清中Livin阳性表达率为52.8%(19/36),正常健康人血清中未见阳性。Livin在低分化腺癌中阳性率为69.6%(16/23),与高、中分化腺癌23.1%(3/13)相比有显著性差异;伴有淋巴结转移者阳性率为68.0%(17/25),无淋巴结转移者阳性率为18.2%(2/11),两者有显著性差异;伴有浆膜层浸润者阳性率为70%(14/20),无浆膜层浸润者阳性率为31.3%(5/16),两者比较有显著性差异。TNM分期Ⅲ期的胃癌患者Livin阳性率为91.7%(11/12),显著高于Ⅰ、Ⅱ期者33.3%(8/24)。Livin基因表达与胃癌组织的分化程度、临床分期及淋巴结转移呈正相关。结论血清Livin水平与胃癌的分期和病理组织学类型有关,为术前评估胃癌的TNM分期和淋巴结转移提供可靠依据。Objective To discuss the relationship between serum Livin level and clinical staging of gastric cancer. Methods The preoperative serum Livin level was detected in 36 cases of gastric cancer and 5 cases of normal subjects by ELISA. Results The positive rate of the serum Livin level was 52.8%（ 19/36）in gastric cancer,and that was negative in normal subjects. The Livin positive rate was 68%（ 17/25 ）in the cases with lymph node metastasis and it was 18.2%（2/11 ）in the cases without lymph node metastasis,with a significant difference between the two. The Livin positive rate was 70%（14/20）in the cases with serosal invasion and it was 31.3%（5/16）in the cases without serosal invasion,with a significant difference between the two. The Livin positive rate was 91.7%（11/12）at gastric cancer TNM stage Ⅲ ,significant higher than the stages Ⅰ and Ⅱ （33.3% 8/24）. The Livin expression was positively correlated with gastric cancer tissue differentiation,clinical staging and lymph node metastasis. Conclusion The serum Livin level is related to gastric cancer staging and histological types,which provides a reliable basis for the preoperative evaluation of gastric cancer TNM staging and lymph node metastasis.
