摘 要:目的通过分析不同角蛋白在日光角化病(AK)和基底细胞癌(BCC)中表达情况,寻找两者间细胞分化差异的证据。方法用免疫组化方法研究角蛋白K1和角蛋白K17在AK和BCC中的表达情况。并对结果进行分析比较。结果CK1和CK17在二者的表达均有显著性差异(P<0.001)。CK1在AK中全部阳性表达,并且强阳性率达到23/28(83%);BCC中则仅有少数弱阳性表达,17/29(59%)阴性表达。CK17在AK中少部分(20%)弱阳性表达,24/30(80%)阴性表达;BCC中则阳性率高达21/26(81%),并且部分强阳性表达。CK17在正常皮肤毛囊、皮脂腺阳性表达。结论本研究结果支持AK向鳞状细胞分化,而BCC的瘤体中CK1阴性表达,从分子生物学上解释了BCC中细胞主要处于未分化状态而不会分化成鳞状细胞癌。CK17在BCC中的阳性表达则为BCC主要向毛囊分化的学说提供了又一证据。Objective To analyze the difference of cell differentiation between actinic keratosis (AK) and basal cell carcinoma (BCC). Methods Immunohistochemistry techniques were used to research the expressions of cytokeratin 1 and cytokeratin 17 in 30 cases of AKs and 30 cases of BCCs. Results The expression of CK1 and CK17 showed significant difference between AKs and BCCs(P0.001). The results showed that CK1 was all positive expression and the strong positive rate was 23/28(83%) in AKs. On contrary, CK1 were poorly positive expressed in 40% BCCs, others were negative expressed. The percentage of CK17 expressing cells was 81% in BCCs and 20% in AKs. CK17 was strong expressed in the cell of hair follicle ang glandular sebaceae. Conclusion AKs mainly express CK1 and suggest its differentiation pattern towards epidermal keratinocytes. BCCs consistently express CK17 and suggests that its differentiation pattern towards the cell of the human hair follicle.
