摘 要:目的探讨脑膜瘤的常规磁共振成像(MRI)、磁共振弥散加权成像(DWI)及氢质子磁共振波谱成像(1H-MRS)影像学特点。方法74例行常规MRI检查,其中DWI扫描66例,ADC值测定23例,1H-MRS扫描43例,将肿瘤区与健侧对应部位进行对照分析。结果19例(25.7%)出现肿瘤实质信号不均,41例(55.41%)出现瘤周水肿,在注射Gd-DTPA增强后71例(95.9%)强化,其中52例(73.2%)均匀强化,19例(26.8%)不均匀强化。DWI序列扫描中,50例(75.76%)出现高信号,7例(10.61%)出现等信号,9例(13.63%)出现低信号。ADC数值范围在63.4×10-5-11.6×10-4mm2/s之间,平均约91.06×10-5mm2/s。1H-MRS扫描中出现明确诊断意义波峰36例(83.72%)均不存在明显NAA峰。Cho峰降低2例(5.56%),明显升高23例(63.89%)。8例出现Lac峰,其中4例与囊变同时存在。结论常规MRI扫描结合DWI、ADC值测定对区分脑膜瘤良恶性有一定的帮助;1H-MRS成像对区分脑膜瘤内不同成分、分型及鉴别脑、内外肿瘤等方面提供较准确信息,从而提高膜瘤的影像诊断率。Objective To study the conventional MRI,diffusion weighted imaging(DWI) and the hydrogen magnetic resonance spectroscopy(1H-MRS) findings in meningiomas.Methods 74 patients with meningiomas underwent conventional MRI.66 cases received DWI and 23 cases of them had apparent diffusion coefficient(ADC);43 cases received 1H-MRS.The tumor area and contralateral side were analyzed.Results 19 cases(25.7%) had cystic in the tumors.Peritumoral edema was observed in 41 cases(55.41%).After administration of Gd-DTPA,71 cases(95.9%) had enhancement,52 cases(73.2%) had homogeneous enhancement and 19 cases(26.8%) had inhomogeneous enhancement.With the DWI scanning,50 cases(75.76%) showed hypointense,7 cases(10.61%) showed isointense,9 cases(13.63%) showed hyperintense.The ADC is ranged from 63.4x10^-5 to 11.6x10^-4 mm^2/s,the average is 91.06x10^-5 mm^2/s.36 cases(83.72%) had meaningful spectroscopy;all of them show absence of N-acetylaspartate.The prominent choline-containing compounds were in 23 cases(63.89%),2 cases(5.56%) were decrease.8 cases had lactate peak,and 4 of them had cystic in the meningiomas.Conclusion With the conventional MRI scanning,DWI imaging and ADC can help with differentiating benign and malignant meningiomas.1H-MRS can provide more accurate information on the distinction between meningiomas of different components,classification and identification of internal and external brain tumors.
