原发性肝癌患者HBV血清标志物、前S1抗原与HBV-DNA的关系分析

摘　　要：[目的]了解原发性肝癌(PHC)患者中乙肝病毒(HBV)的感染状况,探讨HBV血清标志物(HBVM)、前S1抗原与HBV-DNA之间的关系。[方法]采用酶联免疫吸附法(ELISA)检测患者血清中HBVM、前S1抗原和荧光定量PCR法检测HBV-DNA载量。[结果]357例PHC患者中,HBV感染率94.96%(339/357),HBsAg阳性率83.75%(299/357);感染模式以HBsAg、抗HBe和抗HBc3项阳性为多见,占52.94%(189/357);前S1抗原、HBV-DNA阳性率分别为50.14%(179/357)、70.59%(252/357);以HBsAg、HBeAg和抗HBc3项阳性模式中前S1抗原、HBV-DNA阳性率最高,分别为73.3%、93.3%。HBV-DNA的阳性率高于前S1抗原(χ2=31.19,P﹤0.001),随着血清HBV-DNA载量增加,前S1抗原阳性率增加。[结论]HBV感染与原发性肝癌关系密切,以HBsAg、抗HBe和抗HBc3项阳性多见;HBV-DNA阳性率高于前S1抗原,检测肝癌患者血清中前S1抗原、HBV-DNA可更全面反映患者体内HBV的活动性复制程度,弥补HBV血清标志物在肝癌检测中的不足。[Objective] To explore the relationship among hepatitis B virus markers （HBVM）,PreS1-Antigen （PreS1-Ag） and HBV-DNA. [Methods] The HBVM,PreS1-Ag and HBV-DNA were detected by ELISA and Fluorescence Quantitative PCR respectively in serum of the patients with primary hepatocellular carcinoma （PHC）. [Results] Among the 357 PHC patients,the total HBV infection rate was 94.96% （339 / 357） and the positive rate of HBsAg was 83.75% （299 / 357）. The positive rate of the combination of HBsAg,Anti-HBe and Anti-HBc was highest among the five HBVM,which amounted to 52.94% （189 / 357）. The positive rates of PreSl-Ag and HBV-DNA were 50.14% （179 / 357） and 70.59% （252 / 357）,re-spectively. The positive rate of the combination of HBsAg,HBeAg and Anti-HBc was 4.20% （15 / 357）,and in this infection mode the positive rate of PreSl-Ag and HBV-DNA were 73.3% and 93.3% ,respectively. With the increased 1eve1 of HBV-DNA,the positive rate of PreSl-Ag also increased. [Conclusion] The infection of HBV is closely related to PHC. This re-search shows that PreSl-Ag and HBV-DNA are sensitive in diagnosing viral replication in patients with PHC and they could be used as proper supplement to conventional test of HBVM.
