摘 要:目的:分析2型糖尿病(DM2)患者糖化血红蛋白(HbA1C)与血清肿瘤标志物间的相关性。方法:用高压液相色谱法检测HbA1C、电化学发光法检测血清甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、糖类抗原15-3(CA15-3)、铁蛋白(Ft)、β2微球蛋白(β2-m),分析112名DM2患者患病时间、HbA1C与肿瘤标志物间的相关性。结果:DM2患者的HbA1C、CA125、CA19-9、Ft的浓度显著高于正常对照组(P<0.05);DM2患者的年龄与AFP、Ft呈显著负相关。患病时间与Ft水平呈显著负相关(P<0.01)。HbA1C水平与CEA、CA125、CA19-9、Ft呈显著正相关(P<0.01,P<0.05,P<0.01,P<0.05)。AFP与CEA、Ft呈显著正相关(P<0.05,P<0.05)。CEA与CA15-3、CA19-9呈显著正相关(P<0.01,P<0.01)。CA125与CA19-9呈显著正相关(P<0.01)。CA15-3与CA19-9呈显著正相关(P<0.01)。结论:DM2患者的血清肿瘤标志物CA125、CA19-9、Ft的水平较正常人组升高。血清肿瘤标志物CEA、CA125、CA19-9、Ft随HbA1C的升高而增加,与患病时间的长短无关。高血糖对血清肿瘤标志物的表达可能有促进作用。Objective To investigate the correlation ship between blood HbA1C and tumor markers levels in patients with type 2 diabetes. Methods Blood glycosylated haemoglobin (HbA1C) (with high-press liquid chromatograply) and serum tumor markers α-fetoprotein (AFP),carcinoembryonic antigen (CEA),carbohydrate antigen 125 (CA125),carbohydrate antigen 15-3 (CA15-3),carbohydrate antigen19-9 (CA19-9),ferritin (Ft) (with CLIA) were determined in 112 cases of type 2 diabetes (DM2) and 38 controls. Results The levels of HbA1C,CA125,CA19-9,Ft in patients were significantly higher than those in controls (P0.05). The duration of disease was significantly positively correlated with the concentration of Ft (P0.01),but was not correlated with AFP,CEA,CA125,CA15-3,CA19-9 levels HbA1C were significantly positively correlated with CEA,CA125,CA19-9,Ft (P0.01,P0.05,P0.01,P0.05). AFP levels were significantly positively correlated with CEA,Ft (P0.05,P0.05). CEA significantly positively correlated with CA15-3,CA19-9 (P0.01,P0.01). CA125 significantly positively correlated with CA19-9 (P0.01). CA15-3 significantly positively correlated with CA19-9 (P0.01). Conclusion The levels of tumor marker CA125,CA19-9,Ft in patients with DM2 were increased. The concentration of CEA,CA125,CA19-9,Ft in DM2 patients increase along with the increase of levels of HbA1C,but it were not affected by duration of the disease. High glucose might promote the expression of tumor markers in patients with DM2.
