摘  要:目的探讨放射线对鼻腔鼻窦黏膜组织形态学的影响,减少放射性鼻窦炎等并发症的发生。方法依条件筛选12例,按放疗前、放疗中期、放疗后3个月、放疗后18个月,分4个阶段按同例同侧的原则采集标本进行扫描电镜观察。结果放疗前纤毛分布均匀、致密、整齐、运动方向一致。放疗中期纤毛排列紊乱、粗细不均、部分脱落或融合、失方向性,传输功能下降。放疗后3个月纤毛大多数呈倒伏状、排列紊乱、部分融合或断裂、脱落或粘连成簇状或顶端呈球状。放疗后18个月见纤毛再生明显呈致密生长,且数量显著增多,排列与方向较整齐并统一,纤毛传输功能进入恢复期。结论放射线对鼻腔鼻窦黏膜组织形态可产生不同程度损伤,同步辅助治疗能有效减低其影响。Objective To investigate the effects of the radiation on the histomorphology of nasal mucosa,and reduce the occurrence of complications such as radiation nasosinusitis. Methods 12 cases were chosen in accordance with the conditions. Specimens were collected for scanning electron microscopy with the principle of the same side on the same sample including four stages ( pre-radiotherapy,medium-term of radiotherapy,3 months after radiotherapy, 18 months after radiotherapy). Results Before radiotherapy, cilia distributed uniformly, densely regularly and moved in the same direction. In medium-term of radiotherapy, cilia arranged in disorder, fell off partly or got meromixis, lost direction and decreased the transmission functions ; the caliber of cilia was uneven. 3 months after radiotherapy, most cilia lodged, arranged disorderly, some got meromixis, fracture, loss or adhesions, or formed the globular clusters. 18 months after radiotherapy, cilia regeneration was evident, the number of cilia significantly increased, the arrangement and direction of cilia were more orderly. The transmission functions went into the recovery period. Conclusion Radiation can produce different degrees of damage to the histomorphology of nasal mueosa, the simultaneous adjuvant treatment can reduce its impact.
