摘  要:目的通过对鼻咽癌放疗中普通常规模拟定位计划靶区临床处方剂量(DCEP,target)与非定位CT治疗计划优化靶区参考剂量(DNCT,target)的比较分析,了解非定位CT-TPS(治疗计划系统)在临床放疗中的应用价值。方法对33例接受^(60)Co外照射治疗鼻咽癌患者的78个鼻咽双侧野及12个鼻前野的普通常规临床模拟定位计划方案(CEP)与非定位CT-TPS优化方案(NCT)进行比较分析,各项参数组间均值差异用配对t检验,并对比值频数分布直方图进行GaussianCurve性偏度与峰度检验及D检验。结果鼻咽双侧野与鼻前野剂量偏差频数分布直方图结果显示,二者拟合曲线均呈正态分布,误差分别为-4.01%±4.06%和-1.01%±2.93%,有统计学差异(P<0.05)。结论非定位CT-TPS在治疗计划优化选择中发挥重要作用,但也要谨防其对临床的误导。Objective To evaluate the value of the NSIC-CT-TPS(NCT,nonsimulation irradiation condition,computer tomogram,treatment planning system)in radiotherapy for nasopharyngeal carcinoma(NPC)by comparing and analyzing the clinical expectation prescription(CEP)dose of treatment planning system(TPS)and the reference dose of the computer tomogram(CT)of non-simulation irradiation condition(NSIC-CT).Methods The difference of the target depth(dcm),tissue-maximum ratio(TMR),target dose(DcGy)and biologically effective dose(BEDcGy)of the lateral facial field(LFF,n=78 fields)and the anterior facial field(AFF,n=12 fields)in NPC patients with AECL-780 C cobalt-60 radiotherapy were recorded and compared by NCT and CEP.Using the histogram of frequent distribution,according to the norm of CEP and ±5%(International Communittee on Radiation Units,ICRU),the differences(d,TMR,D,BED)of CEP and NCT were analyzed,and the frequent conditions were determined with Gaussian-test and D-test.Results Gaussian-test of both frequent curves indicated normal distribution.The deviations between DNCT,target and DCEP,target for LFF(n=78 fields)and AFF(n=12 fields)were-4.01%±4.06%(P0.05)and-1.01%±2.93%(P0.05),respectively.Conclusion NSIC-CT TPS is one of the most important measures to appraise the optimum plan for NPC in radiotherapy,but it should be alerted to mislead the clinical use.
