下段直肠癌低位保肛术后局部复发的临床病理学分析

摘　　要：目的探讨下段直肠癌根治性切除低位保肛术后局部复发的危险因素。方法回顾性分析92例肿瘤下缘距肛缘≤7 cm的下段直肠癌行根治性切除低位保肛患者的临床病理学资料。结果本组直肠癌术后局部复发率为17.39%(15/92)。局部复发与浸润深度、直肠系膜环周切缘、淋巴结转移、肿瘤分化程度及脉管侵袭密切相关(P<0.05)。结论以上几项病理学指标是下段直肠癌根治性切除低位保肛术后局部复发的重要因素。Objective To investigate the risk factors for local recurrence after radical resection in patients with lower rectal carcinoma.Methods The relationship of local recurrence and clinicopathological characteristics of lower rectal carcinoma was retrospectively analylized in 92 patients with lower rectal carcinoma who received total mesorectal excision and conservation of the anus.Results The incidence of local recurrence of rectal carcinoma after radical resection was 16.3%（15/92）.Local recurrence was associated with invasion depth,tumor differentiation,lymph node metastasis,vessel cancerous emboli and circumferential resective margin status.Conclusion The clinicopathological criteria mentioned above are the significant risk factors for local recurrence after curative resection with conservation of the anus in patients with lower rectal carcinoma.
