摘 要:目的探讨宫颈浸润癌患者、宫颈良性病患者及健康女性血清蛋白质组的差异,以期发现诊断宫颈浸润癌的生物学指标。方法采用表面增强激光解吸离子化飞行时间质谱仪(SELDI-TOF-MS)检测宫颈浸润癌患者的血清蛋白质谱图,利用配套软件进行蛋白峰值鉴定、聚类并建立分类树模型。以43例宫颈浸润癌患者、32例宫颈良性病患者及71例健康女性作为训练组,其血清与IMAC-Cu芯片结合检测差异蛋白质,通过软件分析得到分类树模型,以年龄匹配的20例宫颈浸润癌患者、20例宫颈良性病患者及20例健康女性作为测试组进行独立双盲验证。结果共识别出46种差异蛋白,其中由质荷比为3888、7763、6631的3种蛋白构成的分类树模型的分类准确率为95.9%(140/146),敏感度为95.3%(41/43),特异度为98.1%(101/103),经双盲验证其诊断敏感度和特异度分别为90.0%(18/20)和87.5%(35/40)。结论比较蛋白质组学研究发现,3种蛋白质含量与宫颈癌密切相关,有可能成为宫颈癌诊断的生物标志物。Objective To explore and determine the biologic markers for diagnosis of invasive cervical carcinoma by comparing proteomic differences among patients with invasive cervical carcinoma,benign cervical diseases,and healthy people.Methods The serum proteomics patterns of 43 cases of invasive cervical carcinoma,32 cases of benign cervical lesion and 71 healthy subjects were read by surface enhanced laser desorption/ionization-time of flight-mass spectrometry (SELDI-TOF-MS) to screen significantly differential proteins,and to develop the classification tree model for the diagnosis of invasive cervical carcinoma.Twenty cases of invasive cervical carcinoma,20 cases of benign cervical lesions,and 20 healthy subjects were randomly selected at the same period and assigned as test groups for double-blind verification of the model.Results Forty-six differential proteins were identified from the three groups,and the classification tree model formed by 3 proteins (M/Z:3888,7763 and 6631) could be used to identify invasive cervical carcinoma,benign cervical lesion and healthy subjects with an accuracy of 95.9% (140/146),sensitivity of 95.3% (41/43) and a specificity of 98.1% (101/103),respectively.The double-blind test challenged the model with a sensitivity of 90.0% (18/20) and specificity of 87.5% (35/40).Conclusions Three kinds of proteins closely related to invasive cervical carcinoma have been found by comparative proteomics analysis,and they may be used as the biological markers of invasive cervical carcinoma.
