摘　　要：目的通过报道1例经鼻内镜下切除的累及中颅窝的巨大鼻咽纤维血管瘤,回顾并总结该类病变现有的手术方法。方法总结收治的1例有广泛颅底骨质破坏并局限性累及中颅窝的鼻咽纤维血管瘤的临床特点及治疗方法,复习相关文献以了解不同手术方式的特点。结果本患者通过积极的术前准备,在鼻内镜下完整地切除了肿瘤,随访12个月未见肿瘤复发,未出现术中及术后并发症。结论对于累及颅底骨质而颅内侵犯比较局限的鼻咽纤维血管瘤,可以在充分的术前准备的前提下,行鼻内镜下手术切除。Objective To report a case of endoscopic surgery of juvenile nasopharyngeal angiofibroma（JNA）involving the middle cranial fossa,review and summarize the existing surgical approaches.MethodsThe clinical features and therapeutic methods of a case of JNA involving the middle cranial fossa were summarized,and characteristics of different surgical approaches were reviewed.ResultsThe tumor was successfully excised by endoscopic surgery with active preoperative preparation and the patient was free of residual tumor or recurrence and complication after a follow-up period of 12 months.ConclusionAs for JNA with involvment of skull base bone and middle cranial fossa,the endoscopic surgery is effective on the premise of sufficient preoperative preparation.
