血清肿瘤标志物对胃癌诊断及预后判断的临床意义

摘　　要：目的探讨血清肿瘤标志物癌胚抗原(carcinoembryonic antigen,CEA)、糖链抗原125(carbohydrate antigen125,CA125)、糖链抗原199(carbohydrate antigen 199,CA199)对胃癌诊断及预后判断的临床意义。方法采用电化学发光法测定78例胃癌患者、52例良性胃病者、47例健康人血清中CEA、CA125、CA199的水平,比较三组间及胃癌不同分期患者间的差异。结果胃癌组血清CEA、CA125、CA199均水平高于良性胃病组及健康对照组(P<0.05),标志物联合试验较单项检测灵敏度升高,Ⅲ+Ⅳ期胃癌患者肿瘤标志物水平及阳性率均明显高于Ⅰ+Ⅱ期患者(P<0.05)。结论CEA、CA125及CA199在胃癌具有较高的辅助诊断价值,三项标志物联合检测有助于提高胃癌诊断的灵敏度及预后判断。Objective To investigate the clinical significance of serum tumor markers carcinoembryonic antigen（CEA）, carbohydrate antigen 125（CA125）, carbohydrate antigen 199（CA199） in the diagnosis and prognostic judgement of gastric cancer. Methods The senma levels of CEA, CA125, CA199 were detected by electrochemiluminescence method in 78 patients with gastric cancer, 52 patients with benign gastric diseases and 47 healthy controls, comparing the differences among the three groups and differences between different stages in patients with gastric cancer. Results The serum levels of CEA, CA125, CA199 in patients with gastric cancer were obviously higher than those in the group with benign gastric diseases and in the control group （P 〈 0.05）. Moreover,the sensitivities of the combined detections of tumor markers were higher than those of the respective. The serum levels and the positive rates of these three tumor markers in patients with gastric cancer during Ⅲ ＋ Ⅳ stage were obviously higher than those during I ＋ H stage （ P 〈 0.05）. Conclusion The serum levels of CEA, CA125, CA199 are valuable in auxiliary diagnosis of gastric cancer. The combined detection tumor markers can improve the positive rate of diagnosis and the prognostic judgement of gastric cancer.
