睾丸原发恶性淋巴瘤—附21例报道

摘　　要：目的报道２１例睾丸原发性淋巴瘤，并分析其临床表现，形态特征和免疫表型。方法采用ＡＢＣ法对有石蜡标本的１７例进行免疫组化检测。结果患者年龄范围为３４～７６岁，中位年龄６２岁。临床症状和体征除有睾丸不同程度肿大外，无其它特异性的阳性发现。病变部位包括左侧睾丸１１例，右侧９例，双侧１例。组织学上均为弥漫型非何杰金淋巴瘤。１７例的免疫组化结果表明Ｂ细胞来源为５８．８％（１０／１７），Ｔ细胞来源为４１．２％（７／１７）。结论中老年人出现睾丸弥漫性肿大，首先应考虑睾丸淋巴瘤的可能。本病的确诊依赖病理学的诊断。Objective To investigate a group of 21 cases of primary testicular lymphoma and analyze their clinical manifestations,morphological features and immunophenotypes of the lymphoma.Methods ABC method of immunohistochemistry was applied for the examinations of paraffin embedded specimens of 17 cases.Results The age range of the patients was 34 to 76 years old with a medium age of 62. The main presenting symptom was swollen testis without other specific findings. The involved testes were on the left side in 11 cases, right 9 cases and synchronously bilateral in 1 case. Histologically, they were all diffuse non Hodgkin's lymphoma. The results of immuohistochemistry showed that the lymphoma cells in 10 cases were derived from B lymphocyte (58.8%) and in 7 cases from T lymphocyte (41.2%).Conclusions When a diffuse mass of a testis occurs in a patient with middle or old age, the possibility of lymphoma should be considered first. The definite diagnosis of the disease depends on the pathological examination, and immunohistochemistry plays an important role in differential diagnosis.
