CEAmRNA、CK19mRNA、LUNXmRNA在非小细胞肺癌患者外周血微转移中的表达和意义

摘　　要：【目的】研究非小细胞肺癌外周血CEAmRNA、CK19mRNA和LUNXmRNA的表达及联合检测的临床意义。【方法】采用逆转录聚合酶链反应(RT-PCR)检测59例NSCLC患者、31例肺良性疾病患者及20例健康志愿者外周血CK19mRNA、LUNXmRNA及巢式RT-PCR检测CEAmRNA的表达。【结果】59例NSCLC患者外周血CEAmRNA、CK19mRNA、LUNXmRNA阳性率分别为23.7%(14/59)、33.8%(20/59)、57.6%(34/59)。31例肺良性疾病及20例健康志愿者外周血上述三者的阳性率分别为3.2%(1/31)、12.9%(4/31)、0%(0/31);0%(0/20)。统计结果显示:LUNXmRNA与临床分期、细胞分化程度有关,CEAmRNA与病理类型有关。【结论】CEAmRNA、CK19mRNA和LUNXmRNA均可作为检测NSCLC患者外周血微转移的标志物。CEAmRNA对判断病理类型有一定临床意义。LUNXmRNA表达阳性率与肺癌TNM分期、细胞分化程度有关。[Objection]To study the expression and clinical significance of CEAmRNA,CK19mRNA and LUNXmRNA in peripheral bloods of patients with NSCLC.[Methods]RT-PCR was used to detect the LUNXmRNA,CK19mRNA and nested RT-PCR was used to detect CEAmRNA in peripheral bloods of 59 cases of NSCLC,31 cases with benign lung diseases and 20 healthy volunteers were served as the negative controls.[Results]Among 59 peripheral blood samples from NSCLC patients,the expression rate of CEA mRNA,CK19mRNA and LUNX mRNA was 23.7%（14/59）,33.8%（20/59）and 57.6%（34/59）,respectively;In 31 blood samples from patients with benign lung diseases,1 expressed CEA（3.2%）,4 expressed CK19（13.3%）,and 0 expressed LUNX（0%）。 In the 20 healthy voluteers,none was positive for CEAmRNA,CK19mRNA and LUNXmRNA。Statistical analysis indicated that ,LUNX mRNA was correlated with the clinical stage and tumor cell differentiating degrees of the cancer and CEA mRNA was correlated with pathlogical type.[Conclusions]CEAmRNA,CK19 mRNA and LUNXmRNA might be used as molecular marker for the detection of the micrometastasis in NSCLC. CEA mRNA can help to estimate the pathlogical type of NSCLC（P 0.05）and the positive rate of LUNX mRNA in peripheral bloods of NSCLC was correlated with the clinical stage and tumor cell differentiating degrees of the cancer.
