摘　　要：探讨直肠癌旁移行粘膜(TM)的性质及MTS1基因缺失的临床意义。方法:应用粘液组化方法观察直肠癌旁TM的范围,应用免疫组化方法观察MTS1基因产物表达情况,并与正常粘膜及癌组织进行对照。结果:MTS1基因产物在正常直肠粘膜阳性表达率最高,在癌旁TM、癌组织逐渐表达缺失,三组间差异非常显著(P<0.05);MTS1阴性的直肠癌病例,癌旁TM的范围明显高于MTS1阳性的病例(P<0.05)。结论:MTS1基因失活与直肠癌的发生有关,并提示癌旁TM具有一定的恶性潜势。直肠癌旁移行粘膜与直肠癌MTS1表达缺失之间存在一定的相关性,提示MTS1基因突变或缺失是导致癌旁移行性变化的因素之一。如果将两者联合应用于直肠活检,对直肠癌的早期诊断、指导保肛手术及术后治疗具有重要意义。To study the nature of transitional mucosa(TM) adjacent to rectal carcinoma and the clinical significance of MTS1 gene deficit. Methods: Mucinhistochemical methods were used to observe the range of TM adjacent to rectal carcinoma, and immunohistochemical method was employed to ob-serve the expression of MTSl gene product on TM, using normal mucosa and carcinoma tissue as con-trols. Results: The positive expression level of MTSl gene product was the highest in normal rectal mucosa, and gradually declining in TM and carcinoma tissue, significant difference existed among the 3 groups(P<0.05). The extent of TM in MTSl negative cases was larger than that of MTSl positive ca-ses(P<0.05). Conclusions: The inactivation of MTSl gene is related to the genesis of rectal carcinoma, and TM adjacent to rectal carcinoma might possess certain potential of malignancy. In view of the clo-se relationship that exists between the TM and MTSl gene expression deficit of rectal carcinoma, the latter could be one of the causative factors that induce transitional musocal changes adjacent to car-cinoma. Employing simultaneously these 2 parameters in rectal biopsy could be of great help in the ear-ly diagnosis of rectal carcinoma, and in guiding sphincter-saving operations and post-operative adju-vant therapy.
