摘  要:目的观察肾血管性高血压大鼠血浆肿瘤坏死因子-a(TNF-a)在高血压进程中的动态变化,并探讨TNF—a作为脑血管事件预测因子的可能性。方法SD大鼠60只,采用随机数字表法分为2组,高血压组(H组)(n=45),采用双肾双夹法制作肾血管性高血压大鼠模型;假手术组(SO组)(n=15),不上银夹。鼠尾测压仪测定收缩压;术前及术后2、4、8、12、16周舌下静脉取血,酶联免疫吸附测定法检测血浆TNF—a含量:术后16周取脑组织作HE染色,根据脑血管病理结果,将H组大鼠再分为单纯高血压组(SH组)和高血压脑血管病损组(HCVL组)。结果(1)H组大鼠术后血压呈时间依赖性升高,血浆TNF—a也呈时间依赖性升高,任两个时间点之间差异有统计学意义(P〈0.05)。H组大鼠术后血压与同期SO组比较,差异均有统计学意义(P〈0.05)。(2)SH组术后血压有升高趋势,但增幅较缓;而HCVL组血压持续升高,与同组前一时间点比较,差异有统计学意义fp〈0.05):HCVL组术后血浆TNF-a一直持续升高,明显高于同期SH组TNF-a的水平,差异有统计学意义(P〈0.05)。结论TNF-a参与高血压进程,与脑动脉硬化及脑卒中发生有关;持续TNF-a增高对高血压患者发生脑卒中具有一定的预警作用。Objective To observe the dynamic changes of tumor necrosis factor-a (TNF-a) level in the plasma of renovascular hypertensive rats during the development of hypertension and detemine the possibility of TNF-a as an predictor in the cerebral vessel events. Methods Two-kidney two-clip renovascular hypertension was induced in 45 male SD rats as hypertension (H) group; another 15 rats were employed as sham-operated (SO) group. The systolic blood pressure of preheated conscious rats (37 ℃, 15 minutes) was measured by an indirect tail-cuff sphygrnomanometer. The level of TNF-a in the serum was measured by enzyme-linked immunosorbant assay at pre-operation, and 2, 4, 8, 12, 16 w after the operation. Sixteen w after the operation, 2 ram-thick coronal section of brain tissue were stained with hematoxylin-eosin for routine light microsopic observation. Based on the pathological results of cerebral vessels, the H group was subdivided into isolated hypertension (IH) group and hypertensive cerebral vessels lesion (HCVL) group. Results The postoperative blood pressure (BP) and level of TNF-a in the H group were elevated and time-dependent after the operation with significant differences between any 2 time points (P〈0.05). The blood pressure and the level of TNF-a in the H group were remarkly higher than those in the SO group at corresponding periods (P〈0.05). The postoperative BP in the IH group was gradually advanced, while that in the HCVL group kept increasing and significant differences were noted between any 2 time points (P〈0.05). The level of TNF-a in the HCVL group were increased 2 w after operation and strikingly higher than that in the IH group from 4 to 16 w (P〈0.05). Conclusions TNF-a, involving in the development of hypertension, may connect to the happening ofatherosclerosis and stroke. Up-regulative level of TNF-a may play a precaution role in the process of stroke in patients with hypertension.
