6种癌基因蛋白产物在甲状腺癌中的表达及意义

摘　　要：目的：探讨甲状腺癌癌变的机理。方法：应用免疫组化Ｓ－Ｐ法于石蜡切片对４３例甲状腺癌、１７例腺瘤、１９例瘤旁甲状腺组织进行了ｃ－ｅｒｂＢ－２、ｐ２１、ｐ５３、Ｂｃｌ－２、ｃ－ｍｙｃ、ｐ１６癌基因产物的检测。结果：癌组织中均有不同程度１种或１种以上基因产物表达与腺瘤及瘤旁组织间均存在高度显著性差异（Ｐ＜０．０１）。ｃ－ｅｒｂＢ－２、ｐ２１表达随癌组织分化程度增高而增高。ｐ５３表达随癌组织分化程度增高而减少，与ｃ－ｅｒｂＢ－２间呈负相关（Ｐ＜０．０５）。在淋巴结转移病例中ｃ－ｅｒｂＢ－２阳性率增高明显（Ｐ＜０．０５）。结论：６种癌基因均分别参与了甲状腺癌的发生发展过程，癌变的发生可由多种基因相互作用所致。ｃ－ｅｒｂＢ－２、ｐ２１可能在甲状腺癌尤其是乳头状癌的癌变中起重要作用。癌基因产物检测为临床病理诊断、基因治疗。Purpose To study the carcinogenesis of thyroid carcinoma. Methods The expression of c erbB 2, p21, p53, Bcl 2, c myc and p16 in 43 thyroid carcinoma, 17 adenomas and 19 adjacent normal tissues of thyroid tumor were examined by immunohistochemistry. Results The positivity rate was significantly higher in carcinoma than in adenoma and normal tissue( P< 0 01), expression of c erbB 2 and p21 was increased with increasing degree of differentiation of the carcinoma. Expression of p53 was decreased with the increasing degree of differentiation, a significant inverse correlation was found between p53 and c erbB 2( P< 0 05), expression of c erbB 2 was significant higher in cases with lymph node metastasis( P< 0 05). Conclusion The results suggest that all six oncogene proteins play important roles in occurrence and development of thyroid carcinoma. Oncogene c erbB 2 and p21 may play an important role in papillary carcinoma. Oncogene proteins are assayed for help to pathological diagnosis, gence treatment and prognosis.
