摘  要:目的:探讨大肠腺瘤早期癌变指标。方法:用血型抗原A、B、H,凝集素PNA,单克隆抗体Tn1、PCNA、p53等多种标记物,对34例根据形态确诊的大肠腺瘤早期癌变标本进行标记,观察同一瘤体中瘤区与癌变区的差异。结果:A、B血型抗原在瘤区和癌变区的表达无差异。H抗原仅表达于8例癌变区。瘤区和癌变区的Tn1及PNA标记在强度及细胞定位方面有不同程度的差异。癌变区PCNA标记的数量及强度均明显超过瘤区。p53的总表达率为50%,其中5例瘤区亦有少量表达。结论:Tn1、PNA、PCNA。Purpose To search the more objective and reliable signals for detecting the early malignant transformation in colonic adenoma. Methods An immunohistochemical (IHC) study of blood group A, B and H, McAb of Tn1, PCNA, p53 and lectin of PNA was performed in 34 cases of colonic adenoma with cancerous foci diagnosed morphologically by HE section. The difference of the expression of these markers between adenomatous area (AA) and cancerous area (CA) in the same tumor was investigated. Results (1) Blood group H expressed in 8 cases restricted in CA. (2) The localization of Tn1 and PNA was in a random way in CA with more strong expression in staining intensity. (3) The intensity of PCNA expression was significantly strong in CA (P <0 01). (4) p53 product was present in 17 cases (50 5%) in CA, while only 5 cases in AA with few cells weakly stained. Conclusion It is suggested that in ambiguous cases, the combined use of PNA, Tn1, PCNA and p53 for IHC study is helpful in detecting the early malignant foci in colonic adenoma.
