肝硬化分级与血清CA125水平变化的关系

摘　　要：目的探讨肝硬化患者不同Child-Push肝功能分级与血清肿瘤抗原CA125水平的关系。方法采用免疫化学发光法对34名正常人(对照组)和17例慢性乙型肝炎无肝硬化患者和肝硬化Child分级A级、B级、C级的患者各17例检测血清CA125值,并用组间t检验比较慢性乙肝肝功能正常者与对照组、肝硬化不同分级患者血清CA125值的相关性。结果慢性乙型肝炎无肝硬化组血清CA125值[(10.61±5.94)U/ml]与对照组[(9.92±5.43)U/ml]比较差异无统计学意义(P>0.05)。肝硬化组血清CA125水平[(86.78±55.43)U/ml]显著高于正常对照组(P<0.01)。肝硬化A级组血清CA125值[(23.32±4.83)U/ml]显著高于慢性乙肝无肝硬化组及正常对照组(P<0.05);肝硬化B级组[(43.21±10.57)U/ml]、C级组[(239.32±90.83)U/ml]与慢性乙肝肝功能正常者组、对照组比较差异均有统计学意义(P<0.05或P<0.01);肝硬化A级患者与肝硬化B级、C级患者血清CA125值比较差异均有统计学意义(P<0.01)。肝硬化组Child-Push肝功能分级、血清总胆红素和凝血酶原时间与血清CA125呈正相关(r=0.347,0.318,0.329,均为P<0.01),血清白蛋白含量与血清CA125呈负相关(r=-0.309,P<0.01)。结论肝硬化患者血清CA125水平升高程度与肝功能损伤严重程度密切相关,血清CA125可作为肝硬化患者肝功能损害程度的参考指标,其水平高低可作预测预后的参考。血清CA125水平显著升高者应积极行保肝、抗病毒及抗肝纤维化治疗。Objective To investigate the relationship between the different Child-Push classification of liver function and the levels of serum tumor antigen CA125 in cirrhosis patients.Methods Sixty-eight patients with chronic type B hepatitis were divided into no-cirrhosis group and cirrhosis group.The former had 17 patients and the latter,according to cirrhosis Child-Push classification,were divided into grade A,B,C groups with 17 cases respective.In addition,34 healthy people were selected as control group.The level of serum CA125 in all subjects were detected by means of immunohistochemical chemiluminescence.And t-test was used to compare the correlation of the levels of serum CA125 between the no-cirrhosis group and control group as well as cirrhosis patients.Results In the levels of serum CA125,there was no significant difference between the no-cirrhosis group [（10.61±5.94）U/ml] and the control group [（9.92±5.43 ）U /ml] （P0.05）,and significat higher in the cirrhosis group [（86.78±55.43）U/ml] than in the control group [（23.32±4.83）U/ml] （P0.01）,and significant higher in the grade A group than in the no-cirrhotic group and the control group,and were significant differences in the grade B[（43.21±10.57）U/ml],grade C[（239.32±90.83）U/ml] groups than in the no-cirrhotic group and the control group（P 0.05,P 0.01）,and significant differences in the grade A group than in the grade B and grade C groups（P 0.01）.The liver functional classification in cirrhosis group,serum total bilirubin and prothrombin time is correlated positively with the level of serum CA125 （r=0.347,0.318,0.329,all of the value P were0.01） and the level of serum albumin was correlated negative with the level of serum CA125 （r=-0.309,P0.01）.Conclusion The level of serum CA125 in patients with cirrhosis is closely correlated with the injury degree of liver function,and the level of serum CA125 in patients with cirrhosis can be regarded as a reference index for the injury degree of liver function and prognosi
