外周血AFPmRNA、h-TERTmRNA检测对肝细胞肝癌患者预后的意义

摘　　要：目的检测肝细胞肝癌(HCC)患者外周血甲胎蛋白信使核糖核酸(AFPmRNA)、人端粒酶逆转录酶(hTERT)mRNA的表达,探索其在HCC预后中的应用价值。方法FQ-PCR检测60例HCC患者外周血中AFPmR-NA、hTERTmRNA的表达量,并与对照组进行比较。结果HCC患者AFPmRNA、hTERTmRNA的表达量及阳性率均高于对照组(P<0.01)。术后随着时间的推移,其表达数量相应减少,阳性率也明显降低,以1周后下降最为显著(P<0.05)。在肿瘤标志物持续表达阳性的患者中,其术后复发率也明显高于阴性者(P<0.01)。结论外周血中定量检测AFPmRNA、hTERTmRNA的表达可以及时发现患者术后HCC的微转移,预测肿瘤的复发倾向,为术后的合理治疗提供依据。Objective To detect the expression of alpha-fetoprotein messenger ribonucleic acid（AFPmRNA）,human telomerase reverse transcriptase（hTERT） mRNA in peripheral blood of patients with hepatocellular carcinoma（HCC）,and to study their value for the prognosis in HCC.Methods The serum levels of AFPmRNA and hTERTmRNA in 60 patients with HCC were measured by fluorescent quantitation PCR in different time and compared with the control group.Results Both the expression and positive rate of AFPmRNA and hTERTmRNA in patients with HCC were higher than those in control group（P0.01）,which were corresponded to reduce along with time passing,with the most significant decline one week later（P0.05）.HCC patients with persistently positive expression of AFPmRNA and hTERTmRNA had a higher recurrence rate than negative patients（P0.01）.Conclusions Quantitative detection of AFPmRNA and hTERTmRNA in peripheral blood may find out micrometastasis of HCC in time and forecast the recurrence tendency of tumor,which will provide the basis for treatment after operation.
