摘  要:目的:探讨人体脂肪因子、胰岛素抵抗与非酒精性脂肪性肝病(NAFLD)之间的关系。方法:检测NAFLD患者及对照组人群血浆脂肪因子瘦素、脂联素、抵抗素及胰岛素敏感指数,通过logistic回归分析脂肪因子在非酒精性脂肪肝发病中的变化。结果:NAFLD组的体质指数(BMI)及腰臀比(WHR)显著高于正常对照组(P<0.05)。NAFLD组脂联素与正常对照组相比差异无显著性意义(P>0.05),而NAFLD组的甘油三酯、瘦素及抵抗素显著高于正常对照组(P<0.05),NAFLD组的胰岛素敏感指数显著低于正常对照组(P<0.05)。Logistic回归分析发现WHR、抵抗素是非酒精性脂肪肝发病的独立危险因素。结论:WHR、抵抗素在非酒精性脂肪肝发病中具有重要作用。Objective:To evaluate the role of adipokines,insulin resistance in the non-alcoholic fatty liver disease (NAFLD). Methods:The level of leptin,adiponectin,resistin and insulin sensitive index in NAFLD and control detected,and the role of adipokines in NAFLD was anaylzed by logistic regression analysis. Results:The BMI and WHR are significantly higher in the group of NAFLD than those of the control. The level of adiponectin in NAFLD group is similar to the control. The level of TG,lepfin and resistin are significantly higher in the NAFLD group than those in the control. The insulin sensitive index is significantly lower in the NAFLD. group than that in the control. And the logistic regression analysis reveals that WHR and the level of resistin are independence factors in the pathogenesis of NAFLD. Conclusion:WHR and resistin play an important role in the pathozenesis of NAFLD.
