正常颞下颌关节音与其相应下颌位置的研究

摘  要:目的研究正常颞下颌关节音的特点及关节音产生时相应下颌位置,探讨正常关节音产生的机理。方法按照纳入标准从中山大学光华口腔医学院选取26名个别正常青年人,应用BioPAK口颌分析系统(BioJVA颞下颌关节振动分析仪和BioEGN下颌运动轨迹描记仪)对最大开闭口运动时产生的关节音与产生关节音时下颌所处运动轨迹中的相应位置进行记录分析。结果个别正常青年人的颞下颌关节音频谱图呈单峰,振幅和频率四分数位区间分别为0.40~1.08Pa和48~91Hz,左右侧颞下颌音振幅和频率的差异无统计学意义。个别正常受试者行开闭口运动产生关节音时下颌多处于开口阶段,相应下颌位置比值为74%±15%,显示正常颞下颌关节音产生于下颌运动矢状面开口轨迹距牙尖交错位74%±15%最大开口距离处。结论正常颞下颌关节音振幅和频率呈偏态分布,双侧无明显差异,正常颞下颌关节音产生时下颌处于开口运动轨迹的中下段。Objective To analyze the characteristics of temporomandibular joint (TMJ) sounds and the corresponding mandibular position in young subjects with individual normal occlusion. Methods 26 young healthy volunteers with individual normal occlusion from Guanghua School of Stomatology were included. The TMJ sounds and the mandibular positions of subjects were recorded with BioPAK (BioJVA and BioEGN) system by means of electrovibratograghy and electrognathography respectively. Results The inter-quartile range of the amplitude of TMJ sounds was between 0.40 1.08 Pa. The inter-quartile range of the frequency of TMJ sounds was between 48 - 91 Hz. No significant difference was found between bilateral TMJs for neither the amplitude nor the frequency of TMJ sounds. The mandibular position (indicated by mandibular incisors' tracing path in sagittal plane) was in the 74% ± 15% portion of maximal displacement from ICP in the mandibular opening path when the normal TMJ sounds appeared. Conclusions In youth with individual normal occlusion, the amplitude and frequency of TMJ sounds follows a skewed distribution. The mandibular position is in the middle or lower portion of the mandibular opening path when TMJ sounds appear.
