肝炎后肝硬化并发肺部真菌感染X线表现及鉴别诊断

摘　　要：目的:分析肝炎后肝硬化并发肺部真菌感染的X线表现,提高实际工作中的认识和诊断水平,并进行相应的鉴别诊断。方法:回顾性分析经临床和实验室检查证实的38例肝炎后肝硬化并发肺部真菌感染患者的X线表现,分析其影像表现特点及其发展变化。结果:肺部真菌感染的X线表现按其分布及形态归纳如下:支气管肺炎型8例,节段型或播散型肺炎11例,大叶性肺实变4例,两肺絮团状增浓模糊影10例,圆形或肿块形5例。结论:X线检查对肝炎后肝硬化并发肺部真菌感染的诊断及鉴别诊断有一定价值,同时,动态观察有利于提高诊断的正确性。Objective： To analyse the X-ray image features of opportunistic respiratory fungous infections in posthepatitic cirrhosis and to enhance the level of diagnosis and differential diagnosis in clinic.Methods： The X-ray manifestations of 38 cases confirmed by the clinical trails were reviewed.The imaging characteristics,development and changes of them were analysed.Results： According to the distribution and morphology,the X-ray image features of opportunistic fungous infections were summarized as follows： 8 cases of bronchophneumonia,11 cases of segmental or disseminated pheumonia,4 cases of lobar pheumonia,10 cases of the floc＇s fuzzy shadow,5 cases of round or tumour shape.Conclusion： X-ray examination for posthepatitic cirrhosis with opportunistic fungous infections have certain value in the diagnosis and differential diagnosis.At the same time,dynamic observation of imaging feature can enhance diagnosis.
