早期食道癌的低张双对比造影检查的X线诊断价值

摘　　要：目的:研究探讨早期食道癌的影像学诊断及低张双对比造影的价值。方法:收集69例早期食道癌X线低张双对比造影的影像资料和双对比造影的影像资料及临床病理资料,分析发现低张双对比造影检出率最高达到92.75%。糜烂型和乳头型早期癌症易检出,斑块型次之,平坦型X线难显示。结果:癌组织浸润黏膜下层X线低张双对比造影全部检出,浸润黏膜层检出87.5%,原位癌只有37.5%的检出率,而且不能确诊。结论:食道癌的X线检出以低张双对比造影检出率最高,结合内镜检查能准确进行分型,结合CT检查能了解周围浸润情况。Objective To study the image diagnosis of early esophageal cancer and the diagnostic value of hypotonic double contrast examination.Method The X-ray image and pathological data of 69 cases with early esophageal cancer hospitalized between January 2000 and January 2007 were analyzed.They were examed with hypotonic double contrast examination,and doublecontrast radiography.Results Analysis found that the detection rate of hypotonic double contrast exmanination up to 92.75%.Erosive type and papillary-type early cancer could be detected easier,followed by piaque-type,and flat type X-ray hard to show.When the cancer tissue infiltrated mucosal layer,X-ray hypotonic double contrast examination detection rate was 87.5%,carcinoma in situ,only 37.5%,and could not be confirmed.Conclusion Hypotonic double contrast radiography has the highest detection rate in X-ray detection of esophageal cancer,and the pathology can be accurately typed with endoscopy,and the surrounding infiltration can be understood with CT examination.
