摘　　要：目的研究TNF-α单核苷酸多态性与食管癌发生、发展和转移的关系。方法以PCR-RFL方法分析了食管癌病例(n=202)和按性别、年龄频数对照的正常对照者(n=317)TNF-α-308G/A多态,并比较不同基因型与食管癌发生风险、分化程度以及淋巴结转移的关系。结果TNF-α-308G/A基因频率在病例组与对照组中的分布无显著性差异(p>0.05),与食管癌发生风险无关。携带有A(G/A or A/A)等位基因的食管癌患者,其肿瘤恶性程度显著高于G/G基因型(79.8%&56.1%;OR=2.948,95%CI=1.468～5.920;p<0.05),而且,发生淋巴结转移的风险性显著增加(54.0%&34.5%;OR=2.223,95%CI=1.212～4.076;p<0.05)。结论TNF-α-308G/A多态性可能在食管癌的发展和转移中起一定作用。Objective The current study was designed to investigate the polymorphisms of TNF-α in Northern HeNan esophageal squamous cell carcinoma（ESCC） and test the hypothesis which these genetic variation may have impact on the risk,differentiation and LN metastasis of ESCC.Methods A case-control study of 202 ESCC patients and 317 control subjects（matched for age,sex）was conducted to investigate the role of TNF-α-308G/A in ESCC.Genotyping was perform using PCR-RFLP.The adjusted odds ratio（OR）and 95% confidence interval（CI） were computed using logistic regression.Result The distribution of TNF-α-308G/A genotype frequencies have no significant difference between cases and controls（p〉 0.05）.The rate to differentiation in patients with G/A or A/A genotype significantly higher than in patients with G/G（79.8 % 56.1 %;OR=2.948,95%CI=1.468-5.920;p〈 0.05）.Furthermore,The rate to LN metastasis in patients wih G/A or A/A genotype also significantly increased（54.0%34.5%;OR=2.223,95%CI=1.212-4.076;p〈 0.05）.Conclusion The polymorphisms in TNF-α-308G/A may have significant impact on differentiation and LN metastasis of Northern HeNan ESCC.
