摘 要:目的:探讨多层螺旋CT(MSCT)及后处理技术在大肠癌术前诊断和分期的价值。方法:对33例大肠癌患者应用GELightSpeed16层螺旋CT进行一次屏气完成全腹容积扫描,在工作站应用软件后处理技术进行分析,分别获取多平面重组(MPR),最大密度投影(MIP),表面遮盖成像(SSD),透明法显示(Raysum)及CT仿真内镜成像(CTVE),综合多种后处理成像技术与原始扫描轴位图像相结合进行分析,并与肠镜或/和手术病理对照观察。结果:33例大肠癌患者中MSCT大肠成像,共检出34个癌灶,肿瘤检出率为100%、肿瘤浆膜面浸润诊断准确率90.3%(28/31),淋巴结转移准确率87.2%(95/109个),远处脏器转移准确率100%(5/5),显示供血血管准确率100%(32/32),按照改良Duckes分期总准确率为87.9%,其中术前DuckesA期(1/2),DuckesB期(3/5),DuckesC期(20/21),DuckesD期(5/5)。结论:MSCT后处理及各相同性技术对大肠癌的诊断有独特的优越性,对评估肿瘤的可切除性及制定治疗方案具有很高的价值。Objective:To investigate the multi-slice CT(MSCT)and post-processing technology in colorectal cancer diagnosis and preoperative staging.Methods:33 cases of colorectal cancer patients with GE Light Speed 16-slice spiral CT,the completion of a full abdominal breath-hold volume scanning,in the workstation applications software analysis post-processing techniques,namely access to multi-planar reformation(MPR),maximum intensity projection(MIP),surface shaded display(SSD),Raysum and CT virtual endoscopy(CTVE),combination of post-processing technology to scan the original of axial images for analysis,and colonoscopy or surgery observed pathology.Results:Among the 33 cases of colorectal cancer,a total of 34 foci were detected,tumor detection rates were 100%,tumor serosal surface infiltration of diagnostic accuracy of 90.3%(2831),lymph node metastasis accuracy of 87.2%(95109),and distant organ metastasis accuracy of 100%(55),showed that blood vessels supplying the accuracy of 100%(3232),in accordance with the improvement of the overall accuracy of Duckes staging for 87.9%,which Duckes A pre-operative staging 2 cases,Duckes B staging 5 cases,Duckes C staging 21 cases,Duckes D staging 5 cases.Conclusion:MSCT post-processing technologies and the same diagnosis of colorectal cancer have unique advantage,the assessment of tumor resectability and the development of treatment programs have high value.
