摘  要:目的:探讨急性脑梗死(cerebral infarction,CI)患者中性粒细胞(polym0rphonuclear neutrophil,PMN)表面黏附分子(CD11b)的表达量及其临床意义。方法:用流式细胞术,以单克隆抗体为分子探针,对42例急性脑梗死患者和20例健康对照者外周血中性粒细胞黏附分子CD11b的表达量进行检测(以平均荧光强度MFI来表示),观察急性脑梗死患者72h内和第7天的CD11b MFI,并与健康人作对照。结果:患者组72 h内CD11b的表达量高于正常对照组(P<0.05);脑梗死患者第7天CD11b的表达量较第3天为低(P<0.05),与正常对照组比较差异无统计学意义(P>0.05)。神经功能缺损程度严重者,CD11b表达量多(r=0.35,P<0.05)。结论:急性脑梗死患者发病时,血小板被激活。中性粒细胞CD11b的表达上调,可作为CI发生的预测指标;CD11b的表达量与CI的严重程度相关。Objective To investigate the expression of adhesion molecules on polymorphonuclear neutrophil and its clinical implication in patients with acute cerebral infarction (CI). Method Forty - two never treated CI subjects were selected to the study,20 healthy subjects were studied as the control group. Using immunofluorescence phenotyping and flow cytometry,the expression of platelet membrane glycoprotern CD11 b in peripheral blood was measured within 72 hours after onset of acute cerebral infarction and on 7th day after ictus in patients and controls. Results CD11b immunofluorescence intensity of patients platelet was significantly increased within 72 hours, compared with the control group (P 〈 0.05 ). CD11b measurement on 7th day after onset of infarction revealed a comparable value as found in the control group (P 〉 0. 05). MFI of CD11b increased patients with increasing severity degree of dysneuria. Condusion These results indicate that the expression of CD11b on polymorphonuclear neutrophil increases in acute cerebral infarction patients ,and it might play a valuable role in predicting the attack of CI and is closely related to the severity degree of CI.
