术前MSCT在直肠癌TNM分期中的应用价值

摘　　要：目的探讨术前MSCT影像在直肠癌术前诊断分期中的应用价值。方法回顾性分析腹腔镜直肠癌根治术58例,经术后病理证实为直肠癌的术前MSCT影像学资料,同术后病理进行对比,观察手术前后TNM分期的一致性。结果同最终病理结果相比,术前TNM分期中T分期的敏感度为100%(58/58),准确性为87.9%(51/58),N分期的敏感度为83.7%(36/43),准确性为76.7%(33/43),术前T分期结果与术后病理分期结果相比差异无显著性,一致性较好(2=4.027,P>0.05),N分期结果差异无显著性,一致性较好(2=7.356,P>0.05)。结论术前MSCT可以有效判定直肠肿瘤的浸润深度、淋巴结转移和远处转移等情况,较准确地进行临床TNM分期,应用价值较高。Objective To evaluate the value of muhislice spiral computed tomography （MSCT） in preoperative staging of rectal carcinoma. Methods From December 2007 to June 2007, 58 patients with pathologically proved rectal cancer underwent preoperative MSCT. TNM staging was made according to MSCT and compared with pathologic results. Results Compared with the pathological results, preoperative TNM staging according to MSCT for T staging had the sen- sitivity of 100% （58/58） , accuracy of 87.9% （51/58）. N staging had the sensitivity of 83.7% （36/43） and accuracy of 76. 7% （33/43）. Conclusion Preoperative MSCT can effectively determine the rectal tumor infiltration depth, lymph node metastasis and distant metastasis, etc.
