乳腺癌卵巢转移22例临床分析

摘　　要：目的研究乳腺癌卵巢转移患者的临床资料,探讨其临床病程特点。方法根据1991-2009年医科院肿瘤医院22例乳腺癌卵巢转移患者的临床资料,回顾性分析其临床特点。结果诊断乳腺癌时中位年龄43岁,11例患者病理学类型为浸润性导管癌。诊断卵巢转移灶时中位年龄48岁。卵巢转移灶18例由手术确诊,其中7例在行卵巢去势术时发现,72.2%的卵巢转移为双侧。乳腺癌确诊至出现卵巢转移的中位间隔时间为30个月,原发灶分期和中位间隔时间有相关性。卵巢转移确诊后中位生存期为24.5个月,确诊乳腺癌到随访期结束中位生存时间为60.3个月。卵巢转移后患者1年总生存率62%,3年生存率27%,5年生存率13.1%。结论乳腺癌容易发生卵巢转移,转移多发生在双侧卵巢,多数患者发生转移时临床症状不典型,确诊卵巢转移前多数患者有其它部位转移。初诊时的年龄、分期与卵巢转移性乳腺癌预后可能相关。Objective Explore the clinical characteristics of breast cancer patients with ovarian metastases.Methods The medical records of 22 ovarian metastases came from cancer institute hospital,chinese academy of medical sciences,Beijing,between 1991 and 2009,all patients had a history of breast cancer.Reviewed retrospectively was used to explore the clinical characteristics.Results The median age of breast cancer and ovarian metastasis diagnosis was 43 and 48 years respectively.18 cases of ovarian metastases were diagnosed by operation,of which 7 were incidental findings at therapeutic oophorectomy.11 metastases showed ductal patterns easily identifiable as metastases of breast origin,72.2% of the metastases were bilateral.The median interval between the diagnosis of breast carcinoma and the ovarian metastasis was 30 months.The median survival after the diagnosis of ovarian metastasis was 24.5 months.The OS was 60.3 months.The 1,3,and 5-year survival rate was 62%,27%,13.1% respectively.Conclusion Breast is one of the most common primary origins for metastatic ovarian tumors.The metastases are usually bilateral.Most patients are asymptomatic and have extra-ovarian metastasis before ovarian metastasis diagnosed.The age,TNM stage at diagnosis of the primary tumor maybe related to the survival after the diagnosis of ovarian metastasis.
