94例子宫肌瘤剔除术的3种术式临床比较

摘　　要：【目的】评价开腹子宫肌瘤剔除术（TAM）、阴式子宫肌瘤剔除术（TVM）、腹腔镜下子宫肌瘤剔除术（LM）的疗效及优缺点。【方法】回顾分析TAM33例，TVM33例，LM28例的临床资料。【结果]TVM组手术时间为（62．73±3．67）min，手术时间最短，LM组次之，为（94．82±3．81）min，TAM组最长，为（98．79±4．96）min。TVM组和后两组比较均有统计学意义（P〈0．01）。TAM组术后肛门排气时间长于TVM组和LM镜组，有统计学意义（P〈0．01）。TAM组术后住院天数均长于TVM组和LM组，有统计学差异（P〈0．01）。【结论】3种手术方式各有其优缺点，手术方法的抉择应做到因人而异。[Objective]To evaluate the efficacy, advantages and disadvantages of transabdominal myomectomy（TAM）, transvaginal myemectomy（TVM） and laparoscopie myomectomy（LM）. [Methods]The clinical data of 33 cases with TAM（TAM Group）, 33 cases with TVM and 28 cases with LM（LM Group） were reviewed retrospectively. [Results]The operation time of TVM group was the shortest（62.73 ± 3.67rain）, followed by LM Group（94. 82 ± 3. 81min）. The operation time of TAM Group was the longest （98. 79 ± 4. 96min）. There was significant difference in the operation time between TVM Group and the later two groups（ P 〈0.01）. The mean time of bowel function recovery in the TAM Group was longer than that in both of the TVM Group and LM Group, and there was significant difference between TAM Group and the later two groups（ P 〈0.01）. The blood loss in the TAM Group was much more than that in both of the TVM Group and LM Group. The TAM Group had longer postoperative hospital stay than that of the TVM Group and LM Group, and there was significant difference（ P 〈0.01）. [Conclusion]These three surgery methods have both advantages and disadvantages respectively. The choice of the surgery methods is different from person to person.
