表现为色素性紫癜性苔藓样皮炎的蕈样肉芽肿1例

摘　　要：报告1例以色素性紫癜性苔藓样皮炎为表现的蕈样肉芽肿。患者女,55岁。双侧小腿出现红斑、紫红色丘疹10月,泛发、加重伴瘙痒5月。可见肩部、双上臂、双下肢多发暗紫红斑片和斑块,伴轻度苔藓样变和色素沉着。右侧小腿上1/4暗紫红斑片伴苔藓样变处皮损组织病理示:真皮浅层可见异常淋巴细胞浸润并侵入表皮,形成Pautrier小脓疡。免疫组化染色示:LCA、CD3、CD45RO均阳性。诊断:蕈样肉芽肿。A case of mycosis fungoides presented as pigmented purpuric lichenoid dermatitis is reported.A 55-year-old woman presented with deep purple patches and plaques on her lower extremities for 10 months,and the same lesions on the upper extremities and shoulder for 5 months accompanied with itching.Physical examination revealed that deep purple patches and plaques with pigmentation and lichenification.The histopathological examination showed Pautrier′s microabscesses in the epidermis and the lymphocytes infiltration in the epidermis and upper dermis.Immunohistochemical investigation showed that LCA,CD3 and CD45RO were positive.Accordingly mycosis fungoides was diagnosed.
