陈旧性多灶性后极部视网膜色素上皮病变的眼底及眼底荧光血管造影表现

摘　　要：目的观察陈旧性多灶性后极部视网膜色素上皮病变（multifocal posterior pigment epilheliopathy，MPPE）的眼底及眼底血管造影特征性表现。方法回顾分析2001年3月至2008年10月已确诊MPPE患者的临床资料，共43例55只眼。主要分析其主诉，间接或直接检眼镜眼底检查以及后极部彩色眼底照相所见的眼底表现以及FFA特征。结果眼底检查观察到视网膜有程度不同的色素变动及视网膜下增殖膜形成，FFA出现特征性的向下梯形色素条带形成，血管着色、渗漏，视网膜出现无灌注区及新生血管的形成。结论通过眼底荧光血管造影观察到陈旧性MPPE一系列的特征性表现，有助于对此病的理解，给临床诊断及治疗提供帮助。Objective To observe the features of old multifocal posterior pigment epitheliopathy and its FFA imaging.Methods Clinical data of patients with multifocal posterior pigment epithcliopathy who visited our hospital between March 2001 and October 2008 were analyzed.Forty-three cases （55 eyes） of old multifocal posterior pigment epitheliopathy were included in this retrospective study.Complains, indirect and direct ophthalmoscope examination, color fundus photos and ftmdus fluorescence angiography （FFA） were reviewed.Results The ophthalmoscope examination showed retinal pigmentation and subretinal proliferation.In FFA imaging there was typical pigment band, vessel fluorescence staining, leakage and retinal neovascularization.Conelusions Because there is a deficiency of typical feature of old multifocal posterior pigment epitheliopathy, clinical diagnosis is difficult to obtain.Diagnostic errors of old MPPE are often made, and the prognosis is not so good.Sometime MPPE recurrent can happen by these errors.In this study, we find serial typical features of old MPPE by FFA.These typical features could help doctors deeply understanding, correctly diagnosing and effectively curing this disease.
