多发性硬化合并眼运动功能障碍的分析

摘　　要：目的探讨多发性硬化(multiple sclerosis,MS)合并眼运动功能障碍的特征。方法回顾解放军总医院2004年3月至2007年12月间收治的162例确诊或很可能是MS的连续病例,分析相关的眼部表现,总结眼运动功能障碍的特点。结果162例患者中出现眼科症状或发现眼科异常体征占56.8%,仅以眼部表现为首发症状占16.7%,眼部表现主要为视力下降62例(38.3%)和眼运动功能障碍45例(27.8%),后者主要包括眼球震颤、眼球外展受限、眼睑闭合不全和睁眼困难;此外,还有复视15例(9.26%)和13例(8.02%)眼眶或眼球疼痛。结论多发性硬化合并眼运动功能障碍提示了内侧纵束的损害和第三、六对颅神经损害,还可能为Collier′s征。OBJECTIVE To probe the characteristic of ocular motor dysfunctions related to multiple sclerosis. METHODS One hundred and sixty-two cases consecutive patients of multiple sclerosis treated in PLA Genenal Hospital from 2004.3～2007.12 were retrospectively analyzed ,Ocular manifestation was analysised and the charac-teristic of ocular motor dysfunctions related to multiple sclerosis was summarized. RESULTS Of all 162 cases ,56.8% has ophthalmology complaints or abnomal symptoms,and 16. 7% initially present with only ophthalmologic manifestations,Decreased visual acuity（62 cases ,38.3%） and ocular motor dysfunctions（45 cases ,27.8%） are two common ocular manifestation,the later mainly include nystagmus,abducens dysfunction,hypophasis and eyes open difficulty; In addition,there were 15 cases （9.26%）of diplopia and 13 cases （8.02%） of fossa orbitalis or eyeball pain. CONCLUSIONS MS complicated with ocular motor dysfunctions hint medial longitudinal fasciculus and the third or sixth cranial nerve damage,and may also be collier＇s sign. automated perimetry multiple sclerosis AND ocular motor dysfunction
