成人双颌前突患者舌侧正畸的X线头影测量分析

摘　　要：目的:观察经舌侧隐形正畸矫治器治疗的双颌前突的成人患者牙齿、颌骨以及面部软组织的变化情况。方法:16例使用舌侧隐形矫治器的患者,通过治疗前后X线头影测量分析,观察患者牙齿、颌骨以及面部软组织的变化情况。结果:16例使用舌侧隐形矫治器的患者,经过平均2.6年的治疗后,上下颌平面角FH-AB增大(2.51±5.64,P<0.05),患者的上齿槽座角减小SNA(1.30±1.28,P<0.05),同时,伴有下列数据的减小:上下齿槽座夹角ANB(1.64±0.79,P<0.001)、上唇突度E-UI(4.22±2.41,P<0.001)、下唇突度E-LI(4.09±1.90,P<0.001),上切牙唇倾角(9.45±4.62,P<0.001)、下切牙切点至AB平面的距离3.64±3.12,P<0.001)、上下齿槽座点在合平面上垂足的距离(2.58±1.76,P<0.001)、前牙覆(2.38±1.77,P<0.001)、前牙覆盖(2.05±0.52,P<0.001)。结论:患有双颌前突的成人患者,使用隐形正畸矫治器治疗后,可获得良好的牙齿排列、牙颌关系的同时,也可获得满意的口腔以及颜面部的美学效果。Objective To investigate the oral and maxillofacial changes of lingual appliance treating of adult patients with bimaxillary protrusion.Methods A total of 16 adult patients included in the study.The cephalometric analysis was used to investigate the oral and maxillofacial changes.Results 16 patients were treated with lingual appliance for a mean duration 2.6 years.Significant retroclination of the maxillary inciso（r-8.74±4.10,P0.001）and increased of FH-AB angles（2.51±5.64,P0.05）were accompanied by reductions in the SNA angle（1.30±1.281,P0.05）,ANB angle（1.64± 0.79,P0.001）,AB-UI angle（9.45±4.62,P0.001）,the distance between upper and lower lips to E plane（4.22±2.41,4.09± 1.90,P0.001）,lower incisor to AB plane（3.64±3.12,P0.001）,Ao-Bo（2.58±1.76,P0.001）,overjet（2.05±0.52,P0.001）,overbite（2.38±1.77,P0.001）.Conclusion The esthetic effects on oral and maxillofacial changes are remarkble during lingual orthodontic treatment of adult patients with maxillary or bimaxillary protrusion.
