摘　　要：目的探讨冷循环射频消融治疗老年人肝癌并发症的防治措施。方法对40例老年肝癌患者进行射频消融治疗时,通过对麻醉方法、介导方式、射频电极针的选择,在超声精确介导下选择适宜的进针路径及消融方法,以防止并发症。结果32例患者病灶血供完全消失,射频消融彻底;2例出现皮肤灼伤,无出血、胆管损伤、胆瘘、针道种植等并发症。结论选择合适的麻醉方法是提高疗效、减少并发症的前题;恰当的选择介导方式是防止胃肠穿孔、膈肌损伤的有效方法;选择合适的射频电极针可有效控制射频范围,减少胆管损伤;在超声精确介导下选择避开血管及胆管的进针路径可有效的防止出血、胆管损伤与胆瘘。Objective To investigate the prevention and cure methods for complications of gerontism hepatic carcinoma with cold circulative radio-frequency ablation.Methods Ultrasonic guided cold circulative radio-frequency ablation was used to treat 40 cases of gerontism hepatic carcinoma patients.In order to prevent complications,we chosen the suitable anaesthesia,guidance-modus,electrode needles and the proper puncture guidance and ablation methods under precise ultrasonic-guided.Results 32 cases lost tumor’s blood-supply and have ablated completely;2 cases have skin-ambustion,but have no complications,such as,hemorrhage,bile-vessel injury,biliary fistula and needle track implantation.Conclusion Suitable anaesthesia was the precondition to improve curative-effect and reduce complications;proper puncture-guidance could prevent gastric-intestinal perforation and diaphragmatic muscle injury effectively;choose the right electrode needles could effectively control the radiofrequency region and avoid bile-vessel injury;under precise guided of ultrasonic which could avoid blood-vessel and bile-vessel damage,that could prevent hemorrhage,bile-vessel injury and biliary fistula.
