铝碳酸镁对多器官功能衰竭患者应激性溃疡出血的疗效观察

摘　　要：目的:观察铝碳酸镁在多器官功能衰竭(MODS)应激性溃疡的临床效果。方法:将74例MODS患者随机分为治疗组(36例)和对照组(38例),对照组采用应常规奥美拉唑注射液治疗,治疗组患者在此基础上加用铝碳酸镁1000mg加生理盐水100ml鼻饲,每日三次。观察两组患者治疗后48h内胃液中潜血、pH值及胆汁酸浓度和病死率。结果:两组患者治疗后,治疗组24h内止血率高于对照组(P<0.05);48h内治疗组的病死率也低于常规组(P<0.05);同一时段治疗组24h内胃液pH值及胆汁酸浓度改善明显好于常规组,差异有统计学意义(P<0.05)。结论:MODS并应激性溃疡在常规抗酸治疗的基础上加用铝碳酸镁能迅速降低胃液pH值和胆汁酸浓度,有效治疗应激性溃疡出血,改善患者的预后。Objective：To observe the clinical effect of hydrotalcite on multiple organ dysfunction syndrome（MODS）with stress ulcer bleeding and its mechanism.Method：74 patients with MODS stress ulcer bleeding were randomly divided into treatment group（36 cases）and control group（38 cases）.The control group were treated with omeprazole injection.On this basis,The treatment group were treated with Hydrotalcite 1000mg dissolved in 100ml saline by nasal feeding,three times a day.The gastric juice＇s occult blood test,pH,bile acid and mortality were evaluated in 48h treatment.Result：The treatment group hemostasis rate was higher than control group in first 24 h（P〈0.05）,its＇ mortality rate was also lower than the control group in 48 h（P〈0.05）;In the same treatment period,the treatment group gastric juice＇s pH and bile acid were improved significantly than the conventional group in 24h（P〈0.05）.Conclusion：The hydrotalcite combined with anti-acid treatment could rapidly reduce gastric juice＇s pH and bile acid concentration of MODS stress ulcer patients.The treatment could also control the bleeding effectively and improve the patients＇ prognosis.
