摘  要:目的:探讨牙周疾病与早产的关系。方法:200例孕产妇分为牙周病组(74例)和非牙周病组(126例),统计两组早产和足月产的情况。在妊娠20周~24周时收集到138例孕妇的龈沟液,测定龈沟液中肿瘤坏死因子α、基质金属蛋白酶8,9和前列腺素E2的质量浓度。分娩后收集到129例产妇的羊水,测定羊水中肿瘤坏死因子α、基质金属蛋白酶8,9和前列腺素E2的质量浓度。比较牙周病组与非牙周病组早产发生率,早产妇与足月产妇牙龈液和羊水中的肿瘤坏死因子α、基质金属蛋白酶8,9和前列腺素E2的质量浓度。结果:牙周病组中的早产发生率为31.1%,非牙周病组的早产发生率为6.3%,两组差异有统计学意义(P<0.01)。早产妇龈沟液与羊水中的肿瘤坏死因子α、基质金属蛋白酶8,9和前列腺素E2质量浓度高于足月产妇,差异有统计学意义(P<0.01)。结论:牙周病与早产有一定关系,牙周病是引起早产的原因之一。Objective:To study the relation and the mechanism between periodontal diseases and premature delivery. Methods: 200 pregnant lying - in women were divided into periodontal diseases group(74 cases)and non - periodontal diseases group( 126 cas- es) ,and were counted two groups each other preterm infants and term infants. When 138 pregnant lying - in women were 20 - 24 weeks, to gather their gingival crevicular fluid and determine the tumor necrosis factor α(TNFα), matrix metalloproteinase 8,9(MMP - 8,9) and prostaglandin E2(PGE2) levels in gingival crevicular fluid. After labour, gather the amniotic fluid of 129 lying - in wom- en and determine the TNFa, MMP- 8,9 and PGE2 levels. Results: Premature delivery incidence of periodontal diseases group was 31.1%, but that of non - periodontal diseases group was 6.3 %. The difference of two groups have statistic significance ( P 〈 0.01 ). The levels of TNFα, MMP - 8,9 and PGE2 in gingival crevicular fluid and amniotic fluid of preterm infants was significantly higher than those of term infants ( P 〈 0.01 ). Conclusion :The periodontal diseases should be related to premature delivery to some extent and be one of causes for premature labor.
