两种消化道吻合器在食管癌及贲门癌手术中的应用

摘　　要：目的评价国产与进口两种消化道机械吻合器在食管癌及贲门癌手术中的应用价值。方法回顾性分析403例行食管癌及贲门癌手术患者的手术时间、吻合口瘘发生率、吻合口狭窄发生率,其中194例为手工缝合,209例为机械吻合。结果吻合器组手术时间为(159±19)min,手工组手术时间为(211±33)min,差异有统计学意义(P<0.05);吻合器组术后吻合口瘘为0.96%(2/209),手工组术后吻合口瘘为7.73%(15/194),差异有统计学意义(P<0.05);吻合器组术后吻合口狭窄发生率为2.39%(5/209),手工组术后吻合口狭窄发生率为6.70%(13/194),差异有统计学意义(P<0.05)。国产吻合器组手术时间略长于进口组,但无统计学意义(P>0.05);国产与进口两组吻合口瘘发生率为1.00%、0.92%,吻合口狭窄发生率为3%、1.83%,差异均无统计学意义(P>0.05)。结论应用消化道吻合器可减少手术时间与术后并发症,国产与进口两种吻合器无明显差异。Objective To evaluate the value of domestic-made and imported stapling devices in surgery of esophagus and cardia carcinoma. Methods Retrospective analysis was made on the average operation time,the in cidence of anastomotic leakage and the incidence of anastomotic constriction in 403 patients with carcinoma of esophagus or cardia who underwent esophagogastric anastomosis. Among these patients,194 patients were treated by manualment and 209 by staplers in which 100 with the domestically made staplers and 109 with the imported ones. Results The average operation time was（159±19） rain in staplers group and（211±33） min in manualment group（P〈0.05）. The incidence of anastomotic leakage was 0.96% in staplers group and 7.73% in manualment group（P〈 0.05）. The occurrence of anastomotic constriction was 2.39% in staplers group and 6.70% in manualment group（P〈0.05）.Furthermore,there were no statistical differences in the average operation time,the incidence of anastomotic leakage and the incidence of anastomotic constriction between the domestically made stapler group and imported one. Conclusion Stapling devices can shorten operation time and reduce surgical complication. There is no difference in safety and clinical outcome between two groups with the domestically made staplers and the imported ones.
