巩膜环扎术治疗孔源性视网膜脱离术后的眼表改变

摘　　要：目的观察巩膜环扎术治疗孔源性视网膜脱离患者术后的眼表改变。方法对30例(30眼)孔源性视网膜脱离患者行巩膜环扎术,分别于术前3d、术后1周、2周、1个月、3个月行患眼基础泪液分泌试验、泪膜破裂时间、角膜荧光素染色检查,比较术前、术后的检查结果。结果基础泪液分泌试验结果显示,术前3d(12.75±2.96)mm与术后1周(5.55±3.89)mm、2周(8.32±2.85)mm比较,差异有显著统计学意义(F=4.00,P<0.01);泪膜破裂时间结果显示,术后1周(9.02±3.62)s、2周(10.11±4.76)s、1个月(12.35±4.33)s与术前3d(15.65±5.03)s比较,差异有统计学意义(F=2.85,P<0.05);角膜荧光素染色结果显示,术后1周染色阳性眼数与术前3d比较,差异有统计学意义(χ2=4.44,P=0.035)。结论巩膜环扎术会影响眼球表面微环境,术中应尽量减小手术范围和损伤程度,避免不必要的损伤及并发症的发生。Objective To observe the changes of the ocular surface after scleral encircling operation on retinal rhegmatogenous detachment（RRD）.Methods Scleral encircling operation was performed in 30 eyes of 30 patients with RRD,the Schirmer I test,tear film break-up time,corneal fluorescein staining at 3 days before the operation and 1 week,2 weeks,1 month,3 months after the operation were examined,the results were compared before and after operation.Results The Schirmer I test at 3 days before the operation was（12.75±2.96）mm,at 1 week,2 weeks after the operation were（5.55±3.89）mm and（8.32±2.85）mm,respectively;Compared with that at 3 days before the operation,there was significant difference（F=4.00,P0.01）.The tear film break-up time at 3 days before the operation was（15.65±5.03）s,at 1 week,2 weeks,1 month after operation were（9.02±3.62）s,（10.11±4.76）s and（12.35±4.33）s;Compared with that at 3 days before the operation,there was statistical difference（F=2.85,P0.05）.Compared the fluorescein staining positive eyes with 3 days before the operation,there was statistical difference of 1 week after the operation（χ2=4.44,P=0.035）.Conclusion Scleral encircling operation can influence microenvironment of the ocular surface,the surgery extent and injury degree should be reduced in the operation in order to avoid the unnecessary injury and the appearance of complications.
