小牛血去蛋白提取物眼凝胶对Epi-LASIK术后角膜上皮修复的影响

摘　　要：目的观察小牛血去蛋白提取物眼凝胶对机械法准分子激光上皮瓣下角膜磨镶术(epipolis laser in situ keratomileusis,Epi-LASIK)术后角膜上皮修复的影响,评价其有效性与安全性。方法79例(158眼)接受Epi-LASIK的患者随机分为2组,对照组40例(80眼)术后常规给予3g.L-1泰利必妥滴眼液、1g.L-1氟美瞳滴眼液和泪然滴眼液滴眼,每天4次,共7d,之后常规滴用人工泪液和皮质类固醇滴眼液;试验组39例(78眼)给予3g.L-1泰利必妥滴眼液、1g.L-1氟美瞳滴眼液和泪然眼液滴眼,并加用小牛血去蛋白提取物眼凝胶,每天4次,共7d,之后常规滴用人工泪液和皮质类固醇滴眼液。术后随访6个月,定期观察2组患者术后疼痛反应、角膜上皮愈合时间、视力、等效球镜度数及角膜上皮下雾状混浊(haze)情况。结果术后第1天和第2天试验组疼痛程度评分为1.50±0.43和1.03±0.61,对照组为2.02±0.44和1.60±0.57,试验组疼痛程度均明显轻于对照组(均为P<0.05)。试验组角膜上皮愈合平均时间为(3.01±1.36)d,对照组为(4.08±1.59)d,试验组明显短于对照组(t=4.54,P<0.05)。2组患眼术后视力恢复、等效球镜度数和haze情况差异均无统计学意义(均为P>0.05)。结论Epi-LASIK术后短期应用小牛血去蛋白提取物眼凝胶能够减轻术后疼痛症状,加快角膜上皮愈合,并且安全、无毒副作用。Objective To observe the effect of protein-free calf blood extract eye gel on epithelial healing after Epi-LASIK,and to evaluate the efficacy and safety.Methods Seventy-nine patients（158 eyes）performing Epi-LASIK were randomized into experimental and control groups.Forty patients（80 eyes）were treated as control group with 3 g·L-1 loxacin,1 g·L-1 fluorometholone and tears naturale eye drops for 7 days,four times a day,then the conventional artificial tears and corticosteroid eye drops were given.Thirty-nine patients（78 eyes）were treated as experimental group with 3 g·L-1 loxacin,1 g·L-1 fluorometholone,tears naturale eye drops and protein-free calf blood extract eye gel for 7 days,four times a day,then the conventional artificial tears and corticosteroid eye drops were given.The follow-up time was 6 months,the postoperative pain,time of corneal epithelial recovery,visual acuity,spherical equivalent and corneal subepithelial opacity（haze）were examined at special time in two groups.Results The average pain score of experimental group were 1.50±0.43 on the first day and 1.03±0.61 on the second day,the control group were 2.02±0.44 and 1.60±0.57,the experimental group obviously had milder pain,there was statistical difference（P0.05）.The mean time of corneal epithelial recovery were（3.01±1.36）days in experimental group and（4.08±1.59）days in control group,the former was obviously shorter than the latter,there was statistical difference（t=4.54,P0.05）.There was no significant difference in visual acuity,spherical equivalent and haze between two groups（All P0.05）.Conclusion Protein-free calf blood extract eye gel has effectiveness on reducing ocular pain and promoting cornea epithelial healing after Epi-LASIK,the application is safe and has no side effect.
