彩超引导下左锁骨上小淋巴结细针吸取细胞学检查的临床价值

摘　　要：[目的]探讨彩超引导下细针吸取细胞学检查对左锁骨上小淋巴结定性的临床应用价值。[方法]对228例经超声发现左锁骨上长径<1cm淋巴结的恶性肿瘤患者行超声引导下细针吸取行病理细胞学检查。[结果]228例左锁骨上小淋巴结患者中,经超声引导下FNAC检查证实为转移性肿瘤84例,转移性肿瘤检出率为36.84%。淋巴结L/T比值<2转移性肿瘤检出率约44.25%,L/T比值≥2转移性肿瘤检出率12.96%(χ2=17.340,P=0.000)。有血流信号的结节穿刺肿瘤检出率高于无血流信号者(60.61%vs32.82%,P<0.01)。[结论]彩超引导下细针吸取细胞学检查对左锁骨上<1cm淋巴结定性有一定鉴别价值,为临床治疗方案制定提供重要信息。[Purpose] To investigate clinical value of color Doppler flowing imaging （CDFI） guided fine needle aspiration biopsy for small lymph nodes at left supraclavicular. [Methods] Two hundred and twenty eight patients with malignance and ultrasound diagnosed small lymph node （less than 1cm diameter） at left supraclavicular underwent CDFI-guided fine-needle aspiration cytology. [Results] Of 228 patients, 84 patients with metastasis were proved by CDFI-guided fine-needle aspiration cytology, the detection rate was 36.85%. The metastatic detection rate was 44.26% in lymph node L/T ratio2, and 12.97% in L/T ratio ≥2 （χ^2= 17.340,P=0.000）. The tumor detection rate in patients with blood flow signals was significantly higher than that without blood flow signals （60.61% vs 32.82%,P〈0.01）. [Conclusion] CDFI-guided fine-needle aspiration cytology is valuable to differentiate small lymph node metastasis（less than 1cm diameter） at left supraclavicular. It provides important information for clinical treatment.
