膀胱癌患者选择性静脉尿路造影的临床意义

摘  要:目的探讨膀胱肿瘤患者常规行IVU检查的价值及临床意义。方法对2000-01~2007-10间,我院经手术病理诊断为膀胱肿瘤的622例患者的临床检查资料、病理学资料、手术治疗及预后结果,进行回顾性分析。结果622例经病理证实膀胱肿瘤患者中,经IVU发现肾或输尿管异常70例,其中合并上尿路肿瘤6例(阳性率0.96%),2例表现为一侧泌尿系未显影,4例表现为肾盂积水伴输尿管扩张。70例IVU异常病例中,除4例为肾自截外,其余66例B超检查均表现为肾积水伴或不伴输尿管扩张。结论本研究发现,在膀胱肿瘤患者并发上尿路肿瘤的发病率较低,常规行IVU检查浪费医疗资源,增加患者经济负担,可以考虑在膀胱肿瘤患者先行超声检查,发现有肾盂或输尿管异常的患者行IVU检查。Objective To explore the value and the clinical significance of IVU in the conventional bladder cancer patients Methods 622 cases of bladder carcinoma diagnosed pathologically between January 2000 and October 2007 in our hospital were retrospectively analyzed.Results 622 cases confirmed by pathology in patients with bladder cancer,IVU found abnormal renal or ureteral 70 cases,including the merger upper urinary tract tumors 6 cases(positive rate of 0.96 percent),two cases of one side of urinary showed no enhancement,four cases showed hydronephrosis with dilatation of ureter.IVU 70 cases of abnormal cases,in addition to 4 cases of autonephrectomy,the remaining 66 cases of B-ultrasound examination showed hydronephrosis with or without dilatation of ureter.Conclusion Bladder cancer is common urological tumors,preoperative routine inspection trip IVU know whether upper urinary tract tumors and renal functions,This study found that patients with bladder cancer associated with upper urinary tract cancer incidence is relatively low,routine IVU examination wasting medical resources,increase the economic burden on patients,considered in patients with bladder cancer first color Doppler examination found that the renal pelvis or ureter patients with abnormal IVU inspection.
