摘 要:目的应用动态血糖监测系统(CGMS)评估果糖及木糖醇注射液对2型糖尿病(T2DM)患者血糖波动的影响。方法选择稳定期T2DM患者22例,采用自身对照方法 ,随机在第1、2、3天9:00~11:00分别匀速静脉滴注生理盐水200ml、5%果糖注射液200ml或10%木糖醇注射液100ml。同时用CGMS临测血糖的动态变化,比较血糖波动情况。结果生理盐水组、果糖组及木糖醇组的血糖最高值分别为(10.47±2.45)mmol/L、(9.74±2.03)mmol/L和(10.06±1.49)mmol/L、血糖最低值分别为(5.26±1.69)mmol/L、(5.75±1.73)mmol/L和(5.55±1.40)mmol/L、波动最大幅度分别为(5.22±2.36)mmol/L、(3.98±2.57)mmol/L和(4.51±2.10)mmol/L、血糖标准差为别为(1.66±0.62)、(1.48±0.66)和(1.59±0.60),血糖曲线下面积分别为(1429.16±340.79)mmol·min·L,(1410.82±290.47)mmol·min·L^(-1)和(1449.25±192.28)mmol·min·L。三组间血糖最高值、最低值、波动最大幅度、标准差及吡糖曲线下面积差异均无统计学意义(P>0.05)。治疗过程中均未发现明显不良反应。结论小剂量短期静滴果糖及木糖醇注射液对T2DM患者的血糖波动无明显影响。Objective To investigate the influence of intravenous infusion of fructose and xylitol on blood glucose variability in type 2 diabetic patients. Methods Data of 22 patients (male: female, 18: 4) with type 2 diabetes mellitus were recruited for the study. From 9 AM to 11 AM, all the subjects were administrated one of the three kinds of solutions at constant speed on consecutive days randomly: 0.9% normal saline 200ml, 5% fructose 200ml or 10% xylitol 100ml. The blood glucose concentration was determined at every 5 minutes after infusion by continuous glucose monitoring system (CGMS). Effects of 3 kinds of solution on the variability of blood glucose were compared. A two-tailed P value 40.05 was considered statistically significant. Statistical analysis was carried out with SPSS version 13.0 (SPSS, Chicago, IL, USA). Results The maximum (Max), minimum (Min) of blood glucose, mean amplitude of glycemic excursions (MAGE), standard deviation (SD) and area under curve (AUC) of patients using normal salines, fructose, xylitol were as follows: Max[(10.47 ± 2.45) vs (9.74 ± 2.03) vs (10.06± 1.49)mmol/L], Min[(5. 26±1. 69) vs (5.75±1.73) vs (5.55±1.40)mmol/L], MAGE[(5. 22±2. 36) vs (3.98±2.57) vs (4.51±2.10)mmol/L], SD[(1. 66±0. 62) vs (1.48±0.66) vs (1.59±0.60)land AUC[(1429.16±340.79) vs (1410.82 ± 290.47) vs (1449.25 ± 192.28) mmol / min/L], correspondingly. No significant difference of Max, Min, MAGE, SD or AUC was found between three groups of patients using these 3 kinds of solutions. In addition, no adverse effects were reported. Conclusions Low dosage IV administration of fructose and xylitol for short-term has little effect on blood glucose variability in patients with type 2 diabetes mellitus.
