血清VEGF在鼻咽癌诊断和治疗中作用的Meta分析

摘　　要：目的探讨血清VEGF在NPC诊断、治疗及随访中的作用。方法检索MEDLINE(1966年至今)、EM-BASE数据库(1974年至今)、中国生物医学文献数据库(1979年至今)、中国学术期刊全文数据库检索(1979年至今)、重庆维普数据库(1989至今),手工检索所有纳入试验的参考文献,评价纳入研究的方法学质量,采用RevMan4.2.软件进行Meta分析。结果 Meta分析结果显示,血清VEGF浓度NPC患者明显高于正常人,T4期患者高于T1、T2、T3患者,N3患者高于N0、N1、N2患者,M1患者高于M0患者(P均<0.05);合理治疗后NPC患者血清VEGF浓度明显降低,随访中发现复发者血清VEGF浓度明显增高。结论血清VEGF水平可反映NPC原发灶和淋巴结转移的发展情况,是确定NPC有无远处转移、判断疗效及监测复发的重要标志物。Objective To evaluate the fuction of serum vascular endothelial growth factor in valuing treatment effectiveness,recurrence in follow up and clinical diagnosis of nasopharyngeal carcionma（NPC）.Methods According to the requirements of Cochrane systematic review,a thorough literature search was performed among Chinese Digital Hospital Library（www.chkd.cnki.net）,Chinese Biomedical Literature Disk Database（CBMdisk）and Chinese Scientific Journals Database of VIP（http：//dx2.cqvip.com/）.Data were analyzed by RevMan 4.2 software.Results Serum VEGF concentration in patients with T4,N3,M1 stage was significantly higher than those in T1～T3,N0～N2 and M0 stage,respectively;Serum VEGF significantly decrease after treatment,and increased significantly in patients with recurrent disease.Conclusion Serum VEGF is a biomarker to reflect primary lesion and cervical lymph node metastasis development of NPC,which is also can be a useful tool to detect distant metastasis,treatment effectiveness and tumor recurrence.
