白星技术冷超声乳化术联合折叠式人工晶体植入临床观察

摘　　要：目的观察AMO白星技术冷超声乳化术联合折叠式人工晶体植入治疗白内障的效果.方法对55例(67眼)白内障患者用AMO白星技术冷超声乳化仪,在表面麻醉下行白内障超声乳化术后植入折叠式人工晶体.观察术中超声乳化能量、超声乳化时间、术后反应、术后视力和手术并发症.结果 67眼均在表面麻醉下完成手术.平均超声能量5.13%,平均超声时间64″.术后随访3月,最佳视力≥1.0的术眼为18眼26.86%,95%可信区间16%～38%、最佳视力在0.5～1.0间的为41眼61.19%,95%可信区间49%～73%,最佳视力<0.5的为8眼11.94%,95%可信区间4%～20%.主要并发症:角膜水肿4眼(5.97%),虹膜反应3眼(4.47%).所有患者术中前房稳定,均无切口热损伤.结论白星技术冷超声乳化术联合折叠式人工晶体植入,具有更好、更精确的能量控制,极大地降低了无效能量的使用和减少热量的产生,提高前房稳定性,平静而有效的超声能量使手术效果更为完善,提高了超声乳化白内障吸除手术的效率.Objective To observe the effect of cold phacoemulsification under AMO White Star technology combined with foldable intraocular lens implantation for cataract.Methods 67 eyes of 55 cataract patients were treated with cold phacoemulsification under AMO White Star technology and foldable intraocular lens implantation on the surface anesthesia.Then we observed the intraoperative energy,operative time,postoperative reaction,postoperative visual acuity and complications.Results 67 eyes were performed under surface anesthesia,the mean ultrasonic energy was 5.13% and the mean ultrasound time was 64 seconds.All patients were followed up for 3 months after operation,there were 18 eyes with best visual acuity exceeded 1.0（26.86%）,41eyes with best visual acuity exceeded 0.5 to1.0（61.19%）,and 8 eyes with best visual acuity was lower than 0.5（11.94%）.The main complications：there were 4 eyes with corneal edema（5.97%） and 3 eyes with iris reaction（4.47%）.All patients＇anterior chamber was stable,and no incised heat injury was found.Conclusions Cold phacoemulsification under AMO White Star technology combined with foldable intraocular lens implantation can control energy precisely,greatly reduces the energy use and reduce the ineffective heat generation,increases anterior chamber＇s stability.Effective ultrasonic energy can make the surgery even more perfect,and improve the efficiency of operations.
