532倍频激光联合Nd:YAG激光治疗急性闭角型青光眼后患者角膜内皮的变化

摘　　要：目的观察急性闭角型青光眼患者行532倍频激光联合Nd∶YAG激光治疗后角膜内皮的变化。方法急性闭角型青光眼患者11例20只眼,先以532倍频激光在虹膜面作预光凝,光斑直径300μm,时间0.2～0.3s,功率200～400mW,再用Nd∶YAG激光在原激光斑处透切虹膜,能量1.7～5.0mJ,单脉冲1～2次。治疗前及治疗后1周、1和3个月行角膜内皮检查,观察其大小、形状、密度、数量的变化。结果治疗后1周至1个月与治疗前相比较,角膜内皮平均细胞密度和六边形细胞的百分率下降,平均细胞面积与变异系数增加,差异有显著意义(P<0.05)。治疗后3个月与治疗前比较,角膜内皮平均细胞密度、平均细胞面积、变异系数、六边形细胞的百分率差异无显著意义(P>0.05)。结论532倍频激光联合Nd∶YAG激光治疗急性闭角型青光眼后,早期对角膜内皮有一定的损伤,3个月后角膜内皮的损伤修复。Objective To observe the changes in corneal endothelium after laser surgery using 532 double-frequency laser combined with Nd ∶YAG laser for treatment of acute angle closure glaucoma.Methods Eleven cases（20 eyes）were treated by laser iridectomy. We used the 532 laser to take the prephotocoagulation on iris（light spot 300 μm, time 0.2-0.3 s, power 200-400 mW） and then chose Nd ∶YAG laser cut the laser spot thoroughly （power 1.7-5.0 mJ, monopulse 1-2 sessions）.The corneal endothelium was examined before and 1 day , 1 month and 3 months after laser treatment.Results The corneal endothelial cell density and percentage of hexagonal endothelium were decreased, in the meanwhile the cell area and coefficient of variation were increased. There was significant difference between before laser surgery and 1 week and 1 month after laser surgery but there was without significant difference in corneal endothelial cell density,cell area, coefficient of variation and the percentage of hexagonal endothelium in 3 months after laser treatment.Conclusions The combined therapy could injure the corneal endothelum early but the injury recovers after 3 months.
