肝细胞肝癌患者外周血中AFP-mRNA检测的临床意义

摘　　要：目的探讨甲胎蛋白(AFP)-mRNA在原发性肝癌(HCC)患者外周血中的表达和临床意义。方法采用荧光定量逆转录聚合酶链反应(RT-PCR)方法,检测25例HCC患者外周血中AFP-mRNA的表达。结果HCC患者外周血中AFP-mRNA表达率显著高于肝炎和肝硬化病人(P<0.01);HCC患者外周血中AFP-mRNA表达与病人的肝外转移有关(P<0.05),与病人的临床分期、肿瘤直径、数目、血清中AFP水平无关(P>0.05);HCC患者术后1周外周血AFP-mRNA检出率与术前相比明显降低(P<0.05)。结论AFP-mRNA可作为血循环中有肝癌细胞或肝癌细胞的标志物,手术前后的AFP-mRNA的监测有助于预测肿瘤复发的风险,可用于指导对患者的抗肿瘤治疗。[Objective]To study the expression of AFP （alpha-fetoprotein）-mRNA in the peripheral blood of patients with and hepatocellular carcinoma （HCC） and its clinical significance.[Methods]The expression of AFP-mRNA in the peripheral blood of 25 patients with HCC was tested by fluorescent quantitative RT-PCR.[Results]The expression rate of AFP-mRNA in the peripheral blood of HCC patients was significantly higher than that of patients with hepatitis and liver cirrhosis （P0.01）.The expression of AFP-mRNA in the peripheral blood of HCC patients was related to extrahepatic metastasis （P0.05）,and it was not related to clinical stage,serum AFP level,diameter and number of tumor （P0.05）.The positive rate of AFP-mRNA in the peripheral blood of HCC patients of the first week after operation was significantly lower than that before operation （P0.05）.[Conclusion]AFP-mRNA can be used as the marker of hepatoma cells in blood circulation.The monitoring of AFP-mRNA before and after operation is helpful to predict the risk of tumor recurrence,and it can be used to guide the anti-tumor therapy.
